Radyasyon Hasarının Komplikasyonlarının Yönetiminde Geleneksel ve Tamamlayıcı Tıp
Özet
Bu çalışma, radyasyon hasarının ve kanser tedavilerine (kemoterapi/radyoterapi) bağlı komplikasyonların yönetiminde Geleneksel ve Tamamlayıcı Tıp (GETAT) uygulamalarının yerini modern tıp perspektifiyle incelemektedir. Halsizlik, bulantı, kusma, iştahsızlık, diyare, mukozit ve kserostomi gibi hastanın yaşam kalitesini ve tedaviye uyumunu bozan semptomların hafifletilmesinde GETAT yöntemlerine sıklıkla başvurulmaktadır. Geleneksel Çin Tıbbı (GÇT), akut ve kronik fazları içeren beş farklı radyasyon sendromu tanımlamakta; akupunktur ve bitkisel ekstreler (Chai Hu Shu Gan, Gegenqinlian vb.) vasıtasıyla "çi" (enerji) akışını düzenlemeyi ve organ patolojilerini tedavi etmeyi amaçlamaktadır. Hematolojik yan etkilerde lökosit sayısını artırmak için Zusanli ve Dazhui noktalarına akupunktur uygulanabilirken, bitkisel ilaçlar daha çok kök hücre transplantasyonu sonrası komplikasyonlarda tercih edilmektedir. Pulmoner fibrozis gibi radyasyon ilişkili akciğer hasarlarında proinflamatuar sitokinleri baskılayan ginseng bazlı ekstreler (Shen Mai vb.) kullanılmaktadır. Beyin nekrozu konusunda ise bitkisel tedavilerin etkinliğine dair kanıtlar henüz yetersizdir. Son yıllarda yürütülen çalışmalarda CONSORT ve STRICTA gibi metodolojik standartların benimsenmesiyle klinik araştırma kalitesi artmakta; farmakolojik veri tabanları sayesinde GÇT ve modern tıp entegrasyonu güçlenmektedir. Hekimlerin bu uygulamalar hakkında bilgi sahibi olması, hastaların güvenli ve bilinçli tedavi seçimleri yapabilmesi açısından kritik önem taşımaktadır.
This study evaluates the role of Traditional and Complementary Medicine (TCM/T&CM) in managing radiation injury and complications associated with cancer therapies from a modern medical perspective. T&CM methods are frequently utilized to alleviate symptoms such as fatigue, nausea, vomiting, diarrhea, mucositis, and xerostomia, which significantly impair patients' quality of life and treatment compliance. Traditional Chinese Medicine (TCM) defines five distinct radiation syndromes encompassing acute and chronic phases, aiming to regulate "qi" (energy) flow and treat organ pathologies through acupuncture and herbal extracts like Chai Hu Shu Gan and Gegenqinlian. For hematological side effects, acupuncture can be applied to Zusanli and Dazhui points to increase leukocyte counts, whereas herbal medicines are predominantly preferred for managing complications following hematopoietic stem cell transplantation. In radiation-induced lung injuries like pulmonary fibrosis, ginseng-based extracts (e.g., Shen Mai) that suppress pro-inflammatory cytokines are integrated into care. However, evidence regarding herbal efficacy for radiation-induced brain necrosis remains insufficient. Adopting methodological standards such as CONSORT and STRICTA in recent years has enhanced clinical research quality, while pharmacological databases strengthen the integration of TCM and conventional medicine, enabling physicians to guide patients toward safe, informed therapeutic choices.
