MultiColor (Çok Renkli) Retinal Görüntüleme
Özet
Retina hastalıklarının dökümantasyonu, tanısı ve takibinde yüksek çözünürlüklü fundus görüntüleme önem arz etmektedir. MultiColor görüntüleme; konfokal TLO ile elde edilen yüksek kontrastlı, ‘en-face’ görüntüler ile eşzamanlı optik koherens tomografik değerlendirme de sağlamaktadır. Standart 30°’lik görüntülere ek olarak, özel bir lens yardımı ile 55°’lik fundus fotoğrafı almak da mümkündür. Konvansiyonel renkli fundus fotoğrafına kıyasla daha yüksek çözünürlüklü görüntü sağlamakta, ayrıca SD-OKT ve MultiColor görüntüleme tek cihaz ile yapılabildiği için zaman tasarrufu mümkün olmaktadır. Aniden patlayan bir flaş yerine küçük bir lazer spotu kullanılmakta ve dilate edilmemiş pupilladan görüntü alınabilmektedir. Bu sebeple, çekim anı hasta için daha konforlu olmaktadır. İçeriğindeki üç farklı dalga boyunda lazer sayesinde retinanın farklı derinliklerini net olarak görüntüleyebilmek mümkün olmaktadır. Coğrafik atrofi ve maküla distrofilerinin tanı ve takibinde fundus otofloresans görüntüleme kadar hassas, konvansiyonel fundus fotoğrafına ise üstün bulunmuştur. Yine PKV’nin tipik lezyonu olan poliplerin tanısında özellikle kızılötesi reflektans ile %98’ varan duyarlık oranı bildirilmiştir. Bununla birlikte MultiColor görüntülemenin bazı dezavantajları da bulunmaktadır. Üç farklı reflektansta (mavi, yeşil, kızılötesi) görüntü alması sebebiyle çekim sırasında konvansiyonel renkli fundus fotoğrafına göre uzun fiksasyon ve hasta uyumu gerektirmektedir. Ayrıca renkli fundus fotoğrafı ile elde edilen görüntüler stereoskopik görüntüleme ile oldukça benzer iken, MultiColor görüntüde renkler değişkenlik göstermektedir. Sonuç olarak, MultiColor görüntüleme retinanın farklı katlarını etkileyen birçok patolojinin tanı ve takibinde klinisyenlere değerli bilgiler sağlamaktadır. Günümüzde epiretinal membran, coğrafik atrofi, retinal damar tıkanıklıkları, maküla distrofileri, yaşa bağlı maküla dejeneasyonu ve optik sinir başı patolojilerinin değerlendirilmesinde sıklıkla kullanılmakta ve her geçen gün kullanım alanı genişlemekte olan bir teknolojidir.
