Özet

Ailesel Akdeniz Ateşi (FMF), MEFV genindeki mutasyonlar nedeniyle gelişen, otozomal resesif geçişli kalıtsal ve otoinflamatuvar bir hastalıktır. Özellikle Türk, Yahudi, Arap ve Ermeni popülasyonlarında yaygın olup, dünyada en fazla vaka Türkiye'de görülmektedir. Ataklar halinde seyreden hastalıkta en sık karşılaşılan klinik bulgu %95 oranla şiddetli karın ağrısıdır; bunu 38 °C'nin üzerine çıkan tekrarlayıcı ateş, eklemlerde şişlik ve ağrıyla karakterize akut artrit ile plevrit takip eder. Belirtiler genellikle 20 yaşından önce başlar ve 1 ila 4 gün süren atak dönemlerinde akut faz reaktanlarında belirgin artış gözlenir. Hastalığın en kritik komplikasyonu, organ yetmezliğine yol açabilen ve Türkiye'de en sık FMF kaynaklı gelişen reaktif amiloidozdur (AA tipi). Tanı koydurucu spesifik bir laboratuvar testi olmadığından teşhis temel olarak Tel Hashomer gibi klinik kriterlere dayanır. Tedavinin temelini atak sıklığını azaltan ve amiloidoz gelişimini önleyen sürekli kolşisin kullanımı oluşturur. Kolşisine dirençli %10'luk hasta grubunda ise Anakinra ve Kanakinumab gibi IL-1 blokerleri etkin bir tedavi seçeneği sunar.

Familial Mediterranean Fever (FMF) is an autosomal recessive hereditary autoinflammatory disorder caused by mutations in the MEFV gene. It is highly prevalent among Turkish, Jewish, Arabic, and Armenian populations, with Turkey reporting the highest number of cases globally. Characterized by recurrent attacks, the most frequent clinical manifestation is severe abdominal pain (%95), followed by high fever (>38 °C), acute arthritis with joint swelling, and pleuritis. Symptoms typically manifest before the age of 20, and attacks lasting 1 to 4 days elevate acute phase reactants. The most critical complication is secondary amyloidosis (AA type), which can progress to renal failure and stands as the leading cause of reactive amyloidosis in Turkey. Diagnosis relies primarily on clinical criteria like the Tel Hashomer criteria, as there is no specific diagnostic test. Continuous colchicine therapy remains the primary treatment to reduce attack severity and prevent amyloidosis. For the %10 of patients resistant to colchicine, IL-1 blockers such as Anakinra and Canakinumab serve as vital alternative therapeutic options.
