Gebelik ve Diyabet
Özet
Gebelik döneminde görülen diyabetes mellitus (DM), pregestasyonel diyabet (PGDM) ve gestasyonel diyabet (GDM) olmak üzere ikiye ayrılmaktadır. PGDM, gebelik öncesinde var olan tip 1 ve tip 2 diyabet olgularını kapsarken; GDM ise gebeliğin 24. haftasından itibaren gelişen karbonhidrat intoleransıdır. Kontrolsüz PGDM, fetal anomalilere, makrozomiye ve anne sağlığında ciddi mikrovasküler komplikasyonların ilerlemesine yol açabileceğinden, konsepsiyondan en az 2-3 ay önce kan şekerinin regüle edilmesi ve HbA1c düzeyinin yüzde 6,5'in altında tutulması hedeflenir. Risk faktörlerine bakılmaksızın tüm gebelere 24-28. haftalar arasında tek ya da iki aşamalı oral glukoz tolerans testi (OGTT) ile tarama yapılması önerilir. GDM tespit edilen hastalarda ilk basamak tedavi tıbbi beslenme ve egzersiz yönetimidir. İki haftalık yaşam tarzı değişikliği ile glisemik hedeflere ulaşılamayan durumlarda, plasentayı geçmeyen ve gebelikte güvenli olan insülin tedavisine başlanmaktadır. Metformin gibi oral antidiyabetiklerin kullanımı uzun dönem güvenlik verilerinin yetersizliği nedeniyle ilk tercih değildir. GDM öyküsü olan kadınlarda ilerleyen yıllarda tip 2 diyabet gelişme riski yüksek olduğundan, doğum sonrasındaki 4-12. haftalarda OGTT testi tekrarlanmalı ve uzun dönemde de 1-3 yılda bir düzenli diyabet taraması yapılmalıdır.
Diabetes mellitus detected during pregnancy is classified into pregestational diabetes (PGDM) and gestational diabetes (GDM). PGDM encompasses pre-existing type 1 and type 2 diabetes, whereas GDM is characterized as carbohydrate intolerance emerging after the 24th week of gestation. Uncontrolled PGDM significantly increases maternal risks like preeclampsia and fetal complications, including congenital anomalies and macrosomia. Therefore, blood glucose regulation must be achieved 2-3 months preconception, targetting an HbA1c level of 6.5% or lower. Screening via one- or two-step oral glucose tolerance testing (OGTT) is strongly recommended for all pregnant women between weeks 24 and 28. The primary approach for managing GDM includes medical nutrition therapy and tailored aerobic exercise. If glycemic targets are not met within two weeks of lifestyle modifications, insulin therapy is initiated as the standard pharmacological choice because it does not cross the placenta. Oral antidiabetic agents are not preferred due to insufficient long-term safety data. Postpartum evaluation with a 75g OGTT at 4-12 weeks is crucial since these women carry a higher risk of developing future type 2 diabetes, requiring subsequent screening every 1-3 years.
