Ortodontik Bakış Açısıyla Çocukluk ve Adölesan Dönemlerde Uyku
Özet
Uyku, çocukluk ve adölesan dönemlerindeki bireylerin fiziksel, bilişsel ve duygusal gelişimleri ile akademik başarıları için kritik bir onarım sürecidir. Bu dönemlerde yetersiz uyku, dikkatsizlik, davranışsal problemler ve yaralanma riskinde artışa yol açmaktadır. Ortodontik problemler ile uyku kalitesi arasında doğrudan bir ilişki bulunmakta olup, özellikle çocuklarda üst hava yolu tıkanıklığıyla beliren Obstrüktif Uyku Apnesi (OSA) önemli bir solunum rahatsızlığıdır. OSA; genetik ve çevresel faktörlerden etkilenmekte, geceleri diş sıkma ve ağızdan nefes alma, gündüzleri ise ders başarısızlığı gibi semptomlarla kendini göstermektedir. Teşhis sürecinde klinik muayene, sefalometrik radyografiler ve polisomnografi gibi multidisipliner yöntemlerden yararlanılır. Tedavi seçenekleri arasında pozitif hava yolu basıncı (PAP) ve cerrahi müdahalelerin (adenotonsillektomi) yanı sıra ortodontik yaklaşımlar büyük önem taşır. Ortodontistlerin uyguladığı mandibulayı veya dili öne alan apareyler, damağı stabilize eden düzenekler ve hızlı maksiller genişletme yöntemleri, daralmış çene yapısını düzelterek faringeal hava yolu boyutlarını artırır ve solunum direncini azaltır. Sonuç olarak, diş hekimlerinin rutin muayenelerde uyku bozukluklarını erken tespit etmesi ve tıp hekimleriyle multidisipliner çalışması, çocukların yaşam kalitesini artırmada kritik bir role sahiptir.
Sleep is a critical repair process for the physical, cognitive, and emotional development, as well as academic success, of individuals in childhood and adolescence. Insufficient sleep during these periods leads to inattention, behavioral problems, and an increased risk of injuries. There is a direct relationship between orthodontic problems and sleep quality, and Obstructive Sleep Apnea (OSA), manifested by upper airway obstruction, is a significant respiratory disorder particularly in children. OSA is influenced by genetic and environmental factors, presenting with symptoms such as teeth grinding and mouth breathing at night, and academic failure during the day. Diagnosing this condition requires multidisciplinary methods including clinical examination, cephalometric radiographs, and polysomnography. Along with positive airway pressure (PAP) and surgical interventions (adenotonsillectomy), orthodontic approaches hold great importance among treatment options. Mandibular or tongue advancement devices, palate-stabilizing appliances, and rapid maxillary expansion methods applied by orthodontists correct the narrowed jaw structure, thereby increasing pharyngeal airway dimensions and reducing respiratory resistance. Consequently, the early detection of sleep disorders by dentists during routine examinations and their multidisciplinary collaboration with medical physicians play a critical role in enhancing children's quality of life.
