Özet

Demans, küresel yaşlanma ile birlikte en önemli halk sağlığı sorunlarından biri haline gelmiştir ve dünya genelinde vaka sayısının 2050 yılına kadar 152 milyona ulaşması beklenmektedir. Yaşlanma ve genetik unsurlar (APOE ε4 geni gibi) değiştirilemeyen ana risk faktörleriyken, tüm demans vakalarının yaklaşık %35-40'ını oluşturan 12 adet değiştirilebilir edinsel risk faktörü bulunmaktadır. Çocukluk dönemindeki düşük eğitim seviyesi bilişsel rezervi zayıflatırken; orta yaşta işitme kaybı, hipertansiyon, obezite ve artan alkol tüketimi ön plana çıkmaktadır. İleri yaş döneminde ise sigara kullanımı, depresyon, fiziksel inaktivite, sosyal izolasyon, diyabet ve hava kirliliği riski belirgin şekilde artırmaktadır. Lancet Komisyonu raporlarına göre, özellikle orta yaşta başlayan işitme kaybı en güçlü risk faktörlerinden biridir. Korunma stratejilerinde ise erken yaşta nitelikli eğitimle güçlü bir bilişsel rezerv oluşturmak, zihinsel aktiviteleri sürdürmek, sistolik kan basıncını 130 mmHg'nin altında tutmak, düzenli egzersiz yapmak ve Akdeniz diyetini benimsemek kritik öneme sahiptir. Bireysel yaşam tarzı değişikliklerinin yanı sıra çevre, eğitim ve koruyucu sağlık politikalarının toplumsal düzeyde optimize edilmesiyle demans riskinin %40 oranında azaltılabileceği öngörülmektedir.

Dementia has become one of the most significant public health problems with global aging, and the number of cases worldwide is expected to reach 152 million by 2050. While aging and genetic factors (such as the APOE ε4 gene) are the main non-modifiable risk factors, there are 12 modifiable acquired risk factors that constitute approximately 35-40% of all dementia cases. A low level of education in childhood weakens the cognitive reserve, whereas hearing loss, hypertension, obesity, and increased alcohol consumption come to the fore in midlife. In late life, smoking, depression, physical inactivity, social isolation, diabetes, and air pollution significantly increase the risk. According to the reports of the Lancet Commission, hearing loss starting in midlife is one of the strongest risk factors. Regarding prevention strategies, building a robust cognitive reserve through high-quality education at an early age, maintaining intellectual activities, keeping systolic blood pressure below 130 mmHg, exercising regularly, and adopting the Mediterranean diet are of critical importance. It is anticipated that the risk of dementia can be reduced by 40% through the optimization of environmental, educational, and preventive health policies at the societal level alongside individual lifestyle modifications.
