Özet

Gebelikte tiroid patolojilerinin erken tespiti, maternal ve fetal komplikasyonların önlenmesi açısından kritik bir öneme sahiptir. Hipotiroidi varlığı; preeklampsi, düşük, düşük doğum ağırlığı ve bebekte nörokognitif bozukluk gibi ciddi riskleri beraberinde getirir. Türkiye’nin iyot eksikliği bölgesinde yer alması sebebiyle, gebelik planlayan ve gebe olan tüm kadınlara rutin TSH taraması önerilmektedir. Gebelikte aşikar hipotiroidi sıklığı %0,3-0,5, subklinik hipotiroidi ise %2-3 civarındadır ve en sık nedenleri arasında iyot eksikliği ile otoimmün tiroidit yer alır. Tanıda trimester bazlı TSH referans değerleri esas alınır; yüksek TSH ve düşük sT4 aşikar hipotiroidiyi işaret ederken, normal sT4 varlığı subklinik hipotiroidi olarak tanımlanır. Tedavide, aşikar hipotiroidi saptandığında veya subklinik hipotiroidide anti-TPO antikor pozitifliği olduğunda levotiroksin replasmanına derhal başlanmalıdır. Gebelik öncesi tedavi alan hastaların dozu %30 oranında artırılmalı ve hedef TSH düzeyi 0,5-2,5 mIU/ml arasında tutulmalıdır. İlk 24 haftada 4 haftada bir yapılacak TSH kontrolleri ile yakın takip ve trimester uyumlu doz ayarlaması, sağlıklı bir gebelik süreci için elzemdir.

Early detection of thyroid pathologies during pregnancy is crucial for preventing maternal and fetal complications. The presence of hypothyroidism carries severe risks such as preeclampsia, miscarriage, low birth weight, and neurocognitive deficits in the infant. Since Turkey is an iodine-deficient region, routine TSH screening is highly recommended for all pregnant women and preconception candidates. During pregnancy, the prevalence of overt hypothyroidism is 0.3-0.5%, while subclinical hypothyroidism occurs at 2-3%, with iodine deficiency and autoimmune thyroiditis being the leading causes. Diagnosis relies on trimester-specific TSH reference ranges; elevated TSH with low fT4 indicates overt hypothyroidism, whereas normal fT4 levels signify subclinical hypothyroidism. Regarding management, levothyroxine replacement therapy must be initiated immediately upon diagnosing overt hypothyroidism or in subclinical cases with anti-TPO antibody positivity. For patients previously on levothyroxine, the pre-pregnancy dose should be increased by approximately 30%, targeting a TSH level between 0.5-2.5 mIU/ml. Close monitoring with TSH controls every 4 weeks during the first 24 weeks is essential to ensure trimester-specific dose adjustments and a healthy pregnancy outcome.
