Katastrofik Antifosfolipid Sendromu ile Seyreden Sistemik Lupus Eritematozus (SLE) Vakası
Özet
Sistemik lupus eritamatozus (SLE) özellikle deri, eklemler, kan, böbrekler ve merkezi sinir sistemi olmak üzere vücudun farklı organlarını etkileyebilen kronik, otoimmün bir hastalıktır. Katastrofik antifosfolipid sendromu (KAPS), antifosfolipid sendromunun (APS) nadir görülen ama yaşamı tehdit eden bir formudur ve multiorgan tutulumu ile karakterize olur. Bu yazıda, sistemik lupus eritematozus (SLE) tanısı olan, kısa sürede tromboz ve organ yetmezliği gelişen 35 yaşında kadın bir olgu sunulmuştur. Hastada derin ven trombozu, pulmoner emboli ve böbrek yetmezliği ile birlikte trombositopeni ve yüksek antifosfolipid antikor düzeyleri saptanmıştır. Yoğun antikoagülasyon, pulse steroid, plazmaferez ve intravenöz immünoglobulin (IVIG) tedavisine rağmen yanıt alınamamış, maalesef klinik giderek bozularak hasta hayatını kaybetmiştir. Bu olgu, KAPS’un erken tanı ve multidisipliner yönetiminin önemini vurgulamaktadır.
