Özet
Postpartum kanama (PPK), dünya genelinde maternal mortalite ve morbiditenin önde gelen nedenlerinden biri olup, tüm doğumların %1–10’unu komplike etmektedir. Bu bölümde, PPK açısından yüksek risk taşıyan özel hasta grupları incelenmiştir: anemik gebeler, çoğul gebelikler, obez gebeler, ileri maternal yaş ve adölesan gebelikler. Anemi, azalmış hemodinamik tolerans, koagülasyon bozuklukları ve uterin hipoksi aracılığıyla PPK riskini belirgin artırmaktadır. Çoğul gebeliklerde uterin distansiyon ve sezaryen sıklığının yüksekliği; obez gebelerde bozulmuş myometriyal kontraktilite, artmış sezaryen oranları ve sekonder komplikasyonlar; ileri yaş gebeliklerde obstetrik ko-morbiditeler ve sezaryen oranlarındaki artış; adölesan gebelerde ise immatür üreme organları, anemi ve sosyodemografik faktörler PPK riskini artıran başlıca mekanizmalardır. Yönetim stratejileri arasında antenatal dönemde risk sınıflaması, hematolojik parametrelerin optimizasyonu, profilaktik uterotonik kullanımı, doğumun multidisipliner planlanması ve postpartum yakın izlem öne çıkmaktadır. Her bir grup için bireyselleştirilmiş yaklaşımlar, hem anne sağlığını korumak hem de perinatal sonuçları iyileştirmek açısından kritik öneme sahiptir. Sonuç olarak, PPK’nın önlenmesi ve etkin yönetimi, riskli gruplara yönelik hedeflenmiş stratejiler ve multidisipliner protokoller ile mümkündür.
