Farmakolojik Destek (Profilakizi ve Tedavi)
Özet
Bu bölüm, postpartum kanama (PPK) profilaksisi ve tedavisinde farmakolojik ajanların rolünü kapsamlı şekilde ele almaktadır. Doğumun üçüncü evresinin aktif yönetiminin PPK’nın önlenmesindeki temel adım olduğu vurgulanmış, oksitosin birinci basamak ajan olarak öne çıkarılmıştır. Oksitosinin farmakodinamiği, uygulama yolları (IV/IM) ve doz rejimleri açıklanmış; yüksek doz hızlı IV boluslarınkardiyovasküler yan etkilerine dikkat çekilmiştir. Ayrıca ergometrin, metilergometrin, karboprost, misoprostol, sulproston, traneksamik asit, fibrinojen konsantresi ve rekombinant Faktör VII gibi ek ajanlar tartışılmıştır. Ulusal ve uluslararası kılavuzların profilaksi ve tedavi doz önerileri karşılaştırmalı tablolarla sunulmuştur. Bölüm, kanama kontrolünde basamaklı yaklaşımı ve kombinasyon tedavilerinin avantajlarını vurgularken, yan etki profillerine de dikkat çekmektedir.
