Dermatolojide Tedavi Prensipleri (Topikal Kortikosteroidlerin Akılcı Kullanımı)
Özet
Topikal kortikosteroidler, güçlü antiinflamatuvar etkileri nedeniyle inflamatuvar dermatozların tedavisinde yaygın olarak kullanılan temel tedavi ajanlarıdır. Tedavi etkinliğinin artırılması ve lokal ya da sistemik yan etkilerin en aza indirilmesi; uygun potens, formülasyon, uygulama miktarı ve tedavi süresinin hastaya ve lezyona özgü olarak belirlenmesini gerektirir. Bu bölümde topikal kortikosteroidler potenslerine göre sınıflandırılarak farklı dermatolojik hastalıklardaki endikasyonları ve kanıta dayalı kullanım ilkeleri ele alınmıştır. Formülasyon seçiminde lezyonun anatomik yerleşimi ve özellikleri ile hasta uyumunun önemi değerlendirilmiş; parmak ucu ünitesi kullanılarak uygulanacak ilaç miktarının belirlenmesi, tedavi sıklığı ve süresine ilişkin temel yaklaşımlar açıklanmıştır. Lokal ve sistemik yan etkiler ile bunların gelişimini etkileyen risk faktörlerinin yanı sıra çocukluk dönemi, gebelik ve emzirme gibi özel popülasyonlarda güvenli kullanım ilkeleri tartışılmıştır. Ayrıca steroid fobisi, topikal kortikosteroid bağımlılığı ve yoksunluğu, kombine preparatlar, kullanımın sınırlandırılması gereken klinik durumlar ve kronik inflamatuvar dermatozlarda proaktif tedavi yaklaşımı güncel kanıtlar doğrultusunda ele alınmıştır. Bu çerçevede bölüm, topikal kortikosteroidlerin klinik pratikte etkin, güvenli ve akılcı kullanımına yönelik temel ilkeleri bütüncül bir yaklaşımla sunmaktadır.
