Derinin Bakteriyel Enfeksiyonları
Özet
Bakteriyel deri enfeksiyonları, birinci basamak sağlık hizmetlerinde sık karşılaşılan ve yüzeyel, kendini sınırlayan enfeksiyonlardan yaşamı tehdit eden invaziv tablolara kadar geniş bir klinik spektrum gösteren hastalıklardır. Bu bölümde başlıca gram-pozitif ve gram-negatif bakterilerin neden olduğu deri enfeksiyonları; epidemiyoloji, klinik bulgular, tanı, ayırıcı tanı ve tedavi yaklaşımları açısından ele alınmıştır. İmpetigo, ektima, bakteriyel folikülit, furonkül, karbonkül, apse, selülit ve erizipel gibi sık görülen enfeksiyonların yanı sıra stafilokokkal haşlanmış deri sendromu, toksik şok sendromu, nekrotizan fasiit ve gazlı gangren gibi hızlı tanı ve acil sevk gerektiren tablolar üzerinde durulmuştur. Ayrıca korinebakteriyel enfeksiyonlar, erizipeloid, aktinomikoz ve Pseudomonas ilişkili deri enfeksiyonlarının klinik özellikleri ve tedavi seçenekleri özetlenmiştir. Birinci basamakta doğru klinik tanının konulması, uygun antimikrobiyal tedavinin seçilmesi ve ciddi enfeksiyonu düşündüren kırmızı bayrak bulgularının erken tanınması, komplikasyonların ve mortalitenin azaltılması açısından önem taşımaktadır.
