Derinin Yüzeyel Mantar Enfeksiyonları
Özet
Yüzeyel mantar enfeksiyonları, deri, saç, tırnak ve mukozaları etkileyen ve birinci basamak sağlık hizmetlerinde sık karşılaşılan enfeksiyonlardır. Dermatofitozlar, tinea versikolor, tinea nigra, piedra ve yüzeyel kandidiyazis bu grubun başlıca klinik tablolarını oluşturur. Sıcak ve nemli ortam, aşırı terleme, sıkı kıyafet kullanımı, diyabetes mellitus, immünsüpresyon, ortak eşya kullanımı ve bazı mesleki veya çevresel koşullar enfeksiyon gelişimini kolaylaştıran başlıca faktörlerdir. Tanı çoğu olguda öykü ve klinik muayene ile düşünülebilmekle birlikte, özellikle ayırıcı tanının güç olduğu durumlarda nativ preparat, Wood ışığı, mantar kültürü ve gerektiğinde moleküler yöntemlerden yararlanılabilir. Klinik görünümün enfeksiyonun yerleşim yerine ve etken mikroorganizmaya göre değişebilmesi, bazı tabloların bakteriyel veya inflamatuvar deri hastalıklarıyla karışmasına neden olabilir. Tedavi seçiminde tutulum bölgesi, hastalığın yaygınlığı, nüks ve tedaviye yanıt ile hastanın bağışıklık durumu göz önünde bulundurularak topikal veya sistemik antifungal ajanlar tercih edilir. Özellikle saçlı deri ve tırnak tutulumunda sistemik tedavi gereklidir. Ayırıcı tanının doğru yapılması, gereksiz kortikosteroid kullanımından kaçınılması, predispozan faktörlerin düzeltilmesi ve hijyen önlemlerinin uygulanması tedavi başarısını artırırken nükslerin önlenmesinde de önemli rol oynar. Tanıda güçlük yaşanan, yaygın, tedaviye dirençli, sık tekrarlayan veya immünsüprese hastalarda dermatoloji uzmanına sevk uygun bir yaklaşımdır.
