Akne ve Rozasea
Özet
Akne vulgaris, pilosebase üniteyi etkileyen, sık görülen kronik inflamatuvar bir dermatozdur. Akne vulgaris; foliküler hiperkeratinizasyon, artmış sebum üretimi, Cutibacterium acnes ile ilişkili mikrobiyal değişiklikler ve inflamatuvar mekanizmaların etkileşimi sonucunda gelişir. Klinik olarak komedon, papül, püstül ve nodüllerle seyreder; postinflamatuvar hiperpigmentasyon, eritem ve kalıcı skarlara yol açabilir. Tedavi; hastalığın şiddeti, lezyon tipi, skar riski ve hastanın özelliklerine göre bireyselleştirilir. Topikal retinoidler, benzoil peroksit, antibiyotikler, azelaik asit, sistemik antibiyotikler, hormonal tedaviler ve oral izotretinoin başlıca tedavi seçenekleridir. Rozasea ise özellikle yüzün santral bölgesini etkileyen; geçici veya kalıcı eritem, telenjiektazi, papülopüstüler lezyonlar ve fimatöz değişikliklerle karakterize kronik inflamatuvar bir hastalıktır. Patogenezinde nörovasküler disregülasyon, immün sistem aktivasyonu, Demodex folliculorum, genetik yatkınlık ve çevresel faktörler rol oynar. Tedavide tetikleyicilerden kaçınma ve uygun deri bakımının yanı sıra metronidazol, azelaik asit, ivermektin ve vazokonstriktör topikal ajanlar ile sistemik tetrasiklinler ve seçilmiş olgularda izotretinoin kullanılabilir. Persistan eritem ve telenjiektazilerde lazer ve ışık bazlı tedavilerden yararlanılabilir. Her iki hastalıkta da erken ve uygun tedavi, komplikasyonların ve psikososyal etkilenmenin azaltılması açısından önemlidir.
