Özet

Kronik ağrı, farklı patofizyolojik mekanizmaların rol oynadığı ve çoğu zaman multimodal tedavi yaklaşımı gerektiren kompleks bir klinik durumdur. Adjuvan analjezikler, primer endikasyonları ağrı tedavisi olmamasına rağmen farklı kronik ağrı sendromlarında analjezik etkinlik gösteren ve günümüzde ağrı yönetiminin önemli bileşenleri arasında yer alan ilaçlardır. Bu bölümde kronik ağrı tedavisinde kullanılan başlıca adjuvan ilaç grupları; antidepresanlar, antikonvülzanlar, kortikosteroidler, N-metil-D-aspartat (NMDA) reseptör antagonistleri, alfa-2 adrenerjik agonistler ve intravenöz lidokain başlıkları altında ele alınmaktadır. Bu ilaçların etki mekanizmaları, klinik kullanım alanları, etkinlikleri ve güvenlilik profilleri güncel literatür doğrultusunda değerlendirilmektedir. Ayrıca adjuvan ilaçların nöropatik ağrı, kanser ağrısı ve girişimsel ağrı tedavisindeki rolleri üzerinde durulmaktadır. Uygun hasta ve ilaç seçimiyle adjuvan tedavilerin multimodal ağrı yönetimine katkı sağlayabileceği, tedavinin ağrı mekanizması, komorbiditeler ve olası yan etkiler dikkate alınarak bireyselleştirilmesi gerektiği vurgulanmaktadır.

Chronic pain is a complex clinical condition involving multiple pathophysiological mechanisms and often requiring a multimodal treatment approach. Adjuvant analgesics are medications whose primary indications are not pain management but which provide analgesic efficacy in various chronic pain syndromes and have become important components of contemporary pain management. This chapter reviews the major classes of adjuvant drugs used in chronic pain treatment, including antidepressants, anticonvulsants, corticosteroids, N-methyl-D-aspartate (NMDA) receptor antagonists, alpha-2 adrenergic agonists, and intravenous lidocaine. Their mechanisms of action, clinical indications, efficacy, and safety profiles are discussed in the context of current literature. Particular attention is also given to the roles of adjuvant drugs in neuropathic pain, cancer pain, and interventional pain management. The chapter emphasizes that, with appropriate patient and drug selection, adjuvant therapies can contribute substantially to multimodal pain management and that treatment should be individualized according to the underlying pain mechanism, patient comorbidities, and potential adverse effects.
