Özet

Epidural anestezi ve analjezi uygulamalarında; lokal anesteziklerin etkinliğini artırmak, kümülatif doz gereksinimini sınırlayarak lokal anestezik dozunu ve buna bağlı sistemik toksisite riskini azaltmak, genel anestezi ve sistemik opioid gereksinimini azaltmak, postoperatif analjezi süresini uzatarak daha iyi klinik sonuçlar elde etmek amacıyla çeşitli adjuvan ilaçlardan yararlanılmaktadır. Farklı farmakolojik gruplar adjuvan olarak kullanılabilmektedir.  Bu kitap bölümünde, epidural uygulamalarda kullanılan başlıca adjuvanların etki mekanizmaları, analjezik etkinlikleri, kullanım alanları, dozları ve güvenlilik profilleri güncel literatürler doğrultusunda ele alınmıştır. Mevcut kanıtlar, multimodal analjezinin önemli bir bileşeni olabilecek olan bu adjuvanların doğru seçiminin analjezi kalitesini artırabildiğini, opioid ve lokal anestezik gereksinimini azaltabildiğini ve analjezi süresini uzatabildiğini göstermektedir. Ancak hipotansiyon, bradikardi, sedasyon, bulantı-kusma, kaşıntı ve nörolojik yan etkiler gibi istenmeyen etkiler de ortaya çıkabilmektedir. Epidural adjuvan seçiminde etkinlik ve güvenlilik arasındaki dengenin gözetilmesi, uygun dozların tercih edilmesi ve hastanın klinik özelliklerinin dikkate alınması önem taşımaktadır. Lokal anesteziklerin analjezik etkisini uzatırken sistemik toksisite ve nörotoksisite gibi olumsuz etkileri azaltabilecek ajanlar ve tekniklere yönelik hem yeni ilaçları hem de yeni ilaç taşıma sistemlerini kapsayan araştırmalar devam etmektedir.

While applying epidural anesthesia and analgesia, various adjuvant drugs are used to increase the efficacy of local anesthetics, reduce the required cumulative dose of local anesthetics and in that way, minimize the risk of systemic toxicity, decrease the need for general anesthesia and systemic opioids, and prolong postoperative analgesia to achieve improved clinical outcomes. Various pharmacological substances may serve as adjunctive agents in epidural administration. The present chapter reviews the mechanisms of action, analgesic potency, clinical indications, dosage schemes, and safety profiles of the primary epidural adjuvants based on contemporary literature. Current evidence suggests that the appropriate selection of these adjuvants—a key element of multimodal analgesic strategies—can enhance pain relief quality, diminish opioid and local anesthetic requirements, and extend analgesia duration. Nevertheless, their clinical application carries potential adverse reactions, such as hypotension, bradycardia, sedation, nausea, vomiting, pruritus, and neurological complications. Thus, when choosing epidural adjuvants, it is essential to carefully weigh the balance between efficacy and safety, determine appropriate doses, and individualize the treatment based on the patient's clinical characteristics. Research continues to investigate novel agents and drug-delivery systems aimed at extending the analgesic duration of local anesthetics while minimizing side effects like systemic toxicity and neurotoxicity.
