Akılcı İlaç Kullanımı ve Ağrıya Yaklaşım
Özet
Akılcı ilaç kullanımı (AİK), hastanın klinik gereksinimine uygun ilacın; doğru ilaç, doğru doz, doğru süre, doğru uygulama yolu ve en düşük maliyetle kullanılmasını ifade eder. Ağrı tedavisinde amaç, ağrının nedenini belirlemek, ağrıyı etkili şekilde kontrol altına almak ve hastanın yaşam kalitesini artırmaktır. Tedavi öncesinde ağrının yeri, şiddeti, süresi, tipi ve nedenleri değerlendirilmelidir. Analjezik tedavisinde basamaklı yaklaşım uygulanır. İlaçlar en düşük etkili dozda ve en kısa süreyle kullanılmalı, yan etkiler ve ilaç etkileşimleri düzenli olarak izlenmelidir. NSAİİ'ler gastrointestinal kanama, böbrek hasarı ve kardiyovasküler risk oluşturabileceğinden dikkatli kullanılmalıdır. Opioidler, bağımlılık ve solunum depresyonu riski nedeniyle yalnızca uygun endikasyonlarda ve yakın takip ile kullanılmalıdır. Nöropatik ağrıda adjuvan analjezikler (antidepresanlar ve antikonvülzanlar) tercih edilebilir. Yaşlılar, çocuklar, gebeler ve böbrek-karaciğer hastalarında doz ayarlaması yapılmalı ve tedavi bireyselleştirilmelidir. İlaç tedavisine ek olarak egzersiz, fizik tedavi, sıcak-soğuk uygulama, gevşeme teknikleri ve bilişsel davranışçı terapi gibi farmakolojik olmayan yöntemler de ağrı kontrolüne katkı sağlar. Ağrı yönetiminde akılcı ilaç kullanımı; doğru değerlendirme, uygun ilaç seçimi, güvenli dozlama ve düzenli hasta izlemi ile etkili tedavi sağlarken yan etkileri, ilaç etkileşimlerini ve gereksiz ilaç kullanımını azaltır. Akılcı Tedavi; Ağrının nedenine yönelik olmalıdır. Hastaya özgü planlanmalıdır. En düşük etkili doz tercih edilmelidir. En kısa etkili tedavi süresi uygulanmalıdır. Yan etkiler düzenli olarak izlenmelidir.
Rational drug use (RDU) refers to using the right drug, at the right dose, for the right duration, via the right route of administration, and at the lowest cost, according to the patient's clinical needs. The aim of pain treatment is to identify the cause of the pain, effectively control it, and improve the patient's quality of life. Before treatment, the location, severity, duration, type, and causes of the pain should be evaluated. A stepwise approach is applied in analgesic treatment. Drugs should be used at the lowest effective dose and for the shortest duration, and side effects and drug interactions should be monitored regularly. NSAIDs should be used cautiously as they may cause gastrointestinal bleeding, kidney damage, and cardiovascular risk. Opioids should only be used in appropriate indications and with close monitoring due to the risk of addiction and respiratory depression. Adjuvant analgesics (antidepressants and anticonvulsants) may be preferred in neuropathic pain. Dose adjustment and individualization of treatment should be considered in the elderly, children, pregnant women, and patients with kidney or liver disease. In addition to medication, non-pharmacological methods such as exercise, physical therapy, hot and cold application, relaxation techniques, and cognitive behavioral therapy also contribute to pain control. Rational drug use in pain management provides effective treatment through accurate assessment, appropriate drug selection, safe dosage, and regular patient monitoring, while reducing side effects, drug interactions, and unnecessary drug use. Rational treatment should: Be directed at the cause of the pain. Be planned specifically for the patient. The lowest effective dose should be preferred. The shortest effective treatment duration should be applied. Side effects should be monitored regularly.
