This chapter offers a comprehensive look at exercise-induced delayed onset muscle soreness (DOMS) for athletes, coaches, and health professionals. Tracing the topic from its historical origins to current scientific understanding, it explains the physiological mechanisms behind DOMS, including structural muscle damage, inflammation, disrupted calcium homeostasis, oxidative stress, and neural sensitization, in accessible detail. Readers will also find how individual factors such as age, sex, training status, and genetic predisposition shape DOMS severity, alongside risk profiles specific to sports such as soccer, running, and bodybuilding. Alongside clinical assessment tools like the visual analog scale and biochemical markers, the chapter presents evidence-based prevention and recovery strategies, including gradual loading, warm-up protocols, nutritional interventions, cold-water immersion, and massage, in practical terms. Spanning populations from children to older adults and disciplines from endurance sports to team sports, this chapter serves as an up-to-date, scientifically grounded, and practical guide for anyone seeking to protect athletic health and support sustainable performance.
