Ağrının Halk Sağlığı Boyutu: Epidemiyoloji, Sosyal Belirleyiciler ve Sağlık Sistemine Etkileri
Özet
Ağrı, biyolojik, psikolojik ve sosyal boyutları bulunan, bireysel yaşam kalitesini ve sağlık sistemlerini etkileyen önemli bir halk sağlığı sorunudur. Bu bölümde ağrının epidemiyolojisi, küresel hastalık yükündeki yeri, sosyal belirleyicileri, sağlık eşitsizlikleri ve sağlık sistemi üzerindeki etkileri ele alınmıştır. Kronik ağrı dünya genelinde yetişkinlerin önemli bir bölümünü etkilemekte; özellikle bel ve boyun ağrısı ile osteoartrit, engellilikle geçirilen yaşam yıllarının başlıca nedenleri arasında yer almaktadır. Yaşlanma, sedanter yaşam, obezite, ağır ve ergonomik olmayan çalışma koşulları ile psikososyal stres ağrı yükünü artırmaktadır. Düşük gelir ve eğitim düzeyi, sınırlı sağlık hizmeti erişimi, toplumsal cinsiyet ve kültürel farklılıklar ağrının görülme, ifade edilme ve tedavi edilme biçimini etkilemektedir. Kronik ağrı; doğrudan sağlık harcamalarının yanı sıra iş gücü kaybı, üretkenlik azalması ve rehabilitasyon gereksinimi nedeniyle önemli ekonomik sonuçlar doğurmaktadır. Etkili yönetim için birinci basamağın güçlendirilmesi, multidisipliner ve biyopsikososyal bakım modellerinin yaygınlaştırılması, ergonomik düzenlemeler, fiziksel aktivitenin desteklenmesi ve dezavantajlı gruplarda erişim eşitliğinin sağlanması gerekmektedir. Ulusal ağrı politikaları; önleme, eğitim, erken tanı, rehabilitasyon ve toplum temelli müdahaleleri bütüncül biçimde kapsamalıdır.
Pain is a major public health problem with biological, psychological, and social dimensions that affects individual quality of life and health systems. This chapter reviews the epidemiology of pain, its contribution to the global burden of disease, its social determinants, related health inequalities, and its effects on health services. Chronic pain affects a substantial proportion of adults worldwide, while low back pain, neck pain, and osteoarthritis are among the leading causes of years lived with disability. Population ageing, sedentary lifestyles, obesity, physically demanding or ergonomically inappropriate work, and psychosocial stress contribute to the growing burden. Low income and education, limited access to health care, gender-related factors, and cultural differences influence the occurrence, expression, and treatment of pain. Chronic pain also creates considerable economic costs through direct health expenditures, productivity loss, work absence, and long-term rehabilitation needs. Effective management requires stronger primary care, wider use of multidisciplinary and biopsychosocial care models, ergonomic interventions, promotion of physical activity, and equitable access for disadvantaged populations. National pain policies should integrate prevention, professional and public education, early identification, rehabilitation, and community-based interventions. Such comprehensive approaches may improve quality of life, reduce disability, and support the long-term sustainability of health systems.
