Hasta Güvenliği ve Ağrı Yönetimi
Özet
Günümüzde ağrı yönetimi, semptom kontrolüyle birlikte hasta güvenliği ve hasta merkezli bakım uygulamalarının odağında yer alan bir konu haline gelmiştir. Ağrı yönetiminde hasta güvenliği ile ilgili güncel gelişmeler arasında; multimodal analjezi kombinasyonlarının kullanılması, dijital sağlık teknolojileri, yapay zeka, sanal gerçeklik uygulamaları ve psikososyal müdahale yaklaşımlarının uygulanması, yeni non-opioid analjezikler, transkraniyal manyetik stimülasyon ve transkutanöz elektriksel sinir stimülasyonu gibi non-invaziv nöromodülasyon yöntemlerine yer verilmesi, psikedelik destekli terapilerin kullanılması yer almaktadır. Özellikle yaşlılar, çocuklar, göçmenler, düşük gelirli bireyler, azınlık grupları ve engelliler gibi dezavantajlı hasta gruplarında ağrı yönetimine önem verilmesi, sağlık hizmetlerine erişim, eşitsizliklerin giderilmesi ve hasta güvenliğinin sağlanması açısından önemlidir. Buna ek olarak kaliteli sağlık hizmeti açısından kanıt temelli klinik kılavuzların kullanılması önerilmektedir. Bu kapsamda ulusal ve uluslararası düzeyde sağlık hizmetlerinde kalite ve akreditasyon standartlarında ağrı yönetimi ile ilgili ölçüt ve göstergeler belirlenmiştir. Etkili ağrı kontrolü, hastaların konforunun artırılması, komplikasyonların önlenmesi, ilaç güvenliğinin sağlanması, bakım kalitesinin yükseltilmesi ve sağlık hizmet maliyetlerinin kontrol altına alınması açısından da kritik rol oynar.
Pain management has become a central focus of patient safety and patient-centred care, in addition to symptom control. Recent advances in patient safety related to pain management include the use of multimodal analgesia combinations, digital health technologies, artificial intelligence, virtual reality applications, and psychosocial interventions, as well as the development of novel non-opioid analgesics, non-invasive neuromodulation techniques such as transcranial magnetic stimulation and transcutaneous electrical nerve stimulation, and psychedelic-assisted therapies. Particular attention should be given to pain management in disadvantaged patient populations, including older adults, children, migrants, individuals with low income, minority groups, and people with disabilities, as this is essential for improving access to healthcare, reducing health inequalities, and ensuring patient safety. In addition, the use of evidence-based clinical guidelines is recommended to promote high-quality healthcare. Accordingly, pain management criteria and indicators have been defined in healthcare quality and accreditation standards at the national and international levels. Effective pain control also plays a critical role in improving patient comfort, preventing complications, ensuring medication safety, enhancing the quality of care, and controlling healthcare costs.
