Kabızlık Tedavisinde Cerrahi Yönetim
Özet
Kabızlık, çocukluk çağında en sık karşılaşılan gastrointestinal yakınmalardan biri olup, çocuk gastroenteroloji ve çocuk cerrahisi polikliniklerine başvuruların önemli bir kısmını oluşturmaktadır. Çocuklarda kronik kabızlık yaşam kalitesini olumsuz etkileyen, aile üzerinde psikososyal yük oluşturan ve sağlık hizmeti kullanımını artıran önemli bir sağlık sorunudur. Olguların büyük çoğunluğunu fonksiyonel kabızlık oluştururken, daha küçük bir grupta Hirschsprung hastalığı, anorektal malformasyonlar, nörolojik bozukluklar ve çeşitli metabolik hastalıklar gibi organik nedenler saptanabilmektedir. Fonksiyonel kabızlığın tedavisinde temel yaklaşım; eğitim, davranış düzenlemeleri, diyet değişiklikleri ve farmakolojik ajanları içeren konservatif yöntemlerdir. Bu yaklaşımlar ile hastaların büyük çoğunluğunda başarılı sonuçlar elde edilse de, bir grup hastada tüm konservatif tedavilere rağmen semptomlar devam etmekte ve dirençli kabızlık tablosu gelişmektedir. Dirençli kabızlık olgularında altta yatan patofizyolojik mekanizmanın ayrıntılı olarak değerlendirilmesi ve bireyselleştirilmiş tedavi stratejilerinin planlanması gereklidir. Son yıllarda bağırsak yönetim programlarındaki gelişmeler ve motilite çalışmalarının yaygınlaşması, cerrahi tedavi uygulanacak hasta grubunun daha doğru belirlenmesine katkı sağlamıştır. Bu bölümde kabızlık tedavisinde kullanılan güncel cerrahi yaklaşımlar kapsamlı olarak ele alınacak, uygulanan cerrahi girişimlerin endikasyonları, uygulama yöntemleri, avantajları ve olası komplikasyonları güncel literatür eşliğinde tartışılacaktır.
