Çocuklarda Kabızlık Tedavisinde Non-Farmakolojik Yaklaşımlar

Özet

Bu bölüm, çocuklarda fonksiyonel kabızlığın (FK) non-farmakolojik tedavisini ESPGHAN/NASPGHAN kılavuzu ve güncel literatür kanıtları ışığında ele almaktadır. Eğitim, aile danışmanlığı, fiziksel aktivite, diyet düzenlemeleri ve tuvalet eğitimi, kanıt temeli çoğunlukla kılavuz konsensüsüne ve gözlemsel verilere dayansa da birinci basamağın öne çıkan müdahaleleri olarak değerlendirilmektedir. Buna karşılık biyofeedback, pelvik taban fizyoterapisi ve nöromodülasyon gibi ileri girişimsel yöntemler, sınırlı kanıt düzeyleri nedeniyle ancak refrakter olgularda ve seçilmiş alt gruplarda gündeme gelmektedir. Akupunktur, abdominal masaj ve bitkisel tedaviler gibi tamamlayıcı yaklaşımlar ise umut verici sonuçlar göstermekle birlikte, rutin kullanımı destekleyecek yeterli kanıttan henüz yoksundur. Kabızlığın multifaktöriyel doğası göz önüne alındığında, bireyselleştirilmiş ve multidisipliner bir yaklaşım kaçınılmaz hale gelmektedir; bu doğrultuda bölüm, her müdahalenin kanıt türünü ve öneri gücünü özetleyen tablolarla klinisyenlere günlük pratikte karar desteği sunmayı amaçlamaktadır.
