Organik Kabızlık Sebeplerinin İleri Aşama Tetkikleri

Özet

Kabızlık çocukluk çağında sık görülen bir sorundur. Fonksiyonel ya da organik nedenlere bağlı olabilecek kabızlıkta, çocukların çoğunda semptomdan sorumlu altta yatan bir organik patoloji bulunamaz. Organik patolojiler içinde, Hirschsprung hastalığı, anorektal malformasyonlar, spinal kord anomalileri, endokrin ve metabolik bozukluklar ile yapısal bağırsak patolojileri sayılabilir. Bu durumlarda tanının doğru ve zamanında konulabilmesi için ileri aşama tanısal tetkikler büyük önem taşır. Anorektal manometri, rektal biyopsi, spinal görüntüleme, kontrastlı kolon grafileri, endoskopik incelemeler ve gerektiğinde genetik analizler gibi yöntemler, organik kabızlığın ayırıcı tanısında yol gösterici olmaktadır.  Bu bölümde, çocukluk çağında kabızlık yakınması ile başvuran hastalarda tedaviye dirençli kabızlığın organik nedenlerden ayırt edilmesinde kullanılan ileri tanısal yöntemler ele alınacaktır. Temel değerlendirme yöntemlerinin yetersiz kaldığı olgularda başvurulan laboratuvar, radyolojik, fizyolojik ve invaziv incelemeler sistematik bir yaklaşımla sunularak, organik kabızlığın ayırıcı tanısında doğru, güvenli ve bütüncül bir değerlendirme çerçevesi oluşturulması amaçlanmaktadır.
