Çocuklarda Kabızlığın Psikososyal ve Ailesel Etkileri
Özet
Çocuklarda fonksiyonel kabızlık, sağlık sistemlerinde önemli bir yük oluşturan ve tüm pediatrik başvuruların %3-5'ini kapsayan yaygın bir sorundur. Hastalık yalnızca fiziksel semptomlarla sınırlı kalmayıp çocuğun günlük işlevselliğini de derinden etkilemekte, kabızlıklı çocuklar fiziksel, sosyal, duygusal ve okul işlevselliği alanlarında sağlıklı akranlarına kıyasla anlamlı düzeyde daha düşük yaşam kalitesi göstermektedir. Özellikle dışkı kaçırmanın eşlik ettiği olgularda bu olumsuz etki daha da belirginleşmektedir. Psikolojik açıdan kabızlıklı çocuklarda anksiyete, depresyon, sosyal anksiyete ve içselleştirme-dışsallaştırma türü davranış sorunları sağlıklı akranlara kıyasla anlamlı düzeyde yüksek bulunmuştur. Sosyal boyutta ise okulda tuvalet kullanımına ilişkin kaygı, damgalanma korkusu ve gizleme davranışı sosyal izolasyona yol açabilmektedir. Aile sistemi de bu süreçten ciddi biçimde etkilenmekte; ebeveynlerde depresyon, psikolojik distres ve sağlık sistemiyle yaşanan hayal kırıklıkları belirgin biçimde artmaktadır. Tedavi sürecindeki güçlükler ebeveynlik kimliğini sorgulatabilmekte, kardeşler üzerindeki etkiler ise yeterince araştırılmamış bir alan olarak kalmaktadır. Sonuç olarak, fonksiyonel kabızlığa biyopsikososyal bir çerçeveden yaklaşılması; yaşam kalitesi ölçümünün, psikolojik taramanın ve multidisipliner (hekim, diyetisyen, psikolog, hemşire) bakımın rutin uygulamaya entegre edilmesi önerilmektedir.
