Çocuklarda Kabızlık Nedeni Olan Sistemik Hastalıklar
Özet
Çocukluk çağında kabızlık olgularının büyük çoğunluğu fonksiyonel nedenlere bağlı olmakla birlikte, özellikle erken başlangıçlı, tedaviye dirençli veya sistemik bulguların eşlik ettiği hastalarda organik nedenlerin ayrıntılı olarak araştırılması gereklidir. Sistemik hastalıklarda kabızlık; gastrointestinal motilite bozukluğu, enterik ve otonom sinir sistemi disfonksiyonu, elektrolit ve hormonal dengesizlikler, düz kas fonksiyon bozukluğu, bağ dokusu anomalileri ve nörogelişimsel etkilenme gibi farklı patofizyolojik mekanizmalar sonucunda gelişmektedir. Çocukluk çağında kabızlığa neden olan sistemik hastalıklar, altta yatan hastalığın ilk veya en belirgin klinik bulgularından biri olarak ortaya çıkabilir ve tanıda önemli ipuçları sağlayabilir. Bu nedenle özellikle erken başlangıçlı, tedaviye dirençli veya alarm bulgularının eşlik ettiği olgularda sistemik hastalıkların ayırıcı tanıda mutlaka göz önünde bulundurulması, uygun tanısal değerlendirmelerin yapılması ve altta yatan nedene yönelik tedavinin planlanması büyük önem taşımaktadır. Erken tanı ve uygun tedavi yalnızca kabızlık yakınmalarının düzelmesini sağlamakla kalmayıp, hastalığın ilerlemesini önleyerek çocukların büyüme, gelişme ve yaşam kalitesine de önemli katkı sağlamaktadır.
