Özet

Giriş: Herediter nonsferositik hemolitik anemiler (HNSHA), eritrosit membran bozuklukları ve hemoglobinopatiler dışlandıktan sonra düşünülen, eritrosit metabolizmasındaki enzim kusurlarına bağlı gelişen nadir konjenital hemolitik anemi grubudur. Hekzokinaz-1 (HK1) eksikliği ise glikoliz yolunun ilk basamağında görev alan hekzokinaz enziminin fonksiyon kaybına bağlı gelişen oldukça nadir bir eritrosit enzimopatisidir.

Olgu: On sekiz yaşında kadın hasta, çocukluk çağından itibaren devam eden sarılık yakınması ile başvurdu. Laboratuvar incelemelerinde indirekt hiperbilirubinemi, belirgin retikülositoz ve düşük haptoglobin düzeyi ile uyumlu kronik hemoliz saptandı. Periferik yaymada tanıya özgü eritrosit morfolojisi izlenmedi. Hemoglobin elektroforezi, ozmotik frajilite testi, G6PD ve pirüvat kinaz enzim aktiviteleri normal bulundu; PNH klonu saptanmadı. Klinik ekzom dizi analizinde HK1 geninde homozigot NM_001358263.1.1598T>C (p.Leu533Ser) varyantı gösterildi ve hastaya HK1 eksikliğine bağlı herediter nonsferositik hemolitik anemi tanısı konuldu. Splenomegali ve kolelitiazis nedeniyle uygulanan splenektomi ve kolesistektomi sonrasında hemoliz bulgularında gerileme izlendi.

Sonuç: Kronik otoimmün olmayan hemolitik anemilerde periferik yaymanın tanısal olmayabileceği ve rutin eritrosit enzim analizlerinin normal bulunabileceği, altta yatan sebebinin eritrosit enzimopatileri olabileceği akılda tutulmalıdır. Bu olgu, HK1 eksikliğinde yeni nesil dizileme yöntemlerinin tanısal değerini ve uygun seçilmiş hastalarda splenektominin klinik yarar sağlayabileceğini göstermektedir.
