İleri Yaşta Primer Santral Sinir Sistemi Lenfoması: Olgu Eşliğinde Güncel Yaklaşım
Özet
Primer santral sinir sistemi lenfoması (PSSSL), nadir görülen ve agresif seyirli bir non-Hodgkin lenfoma alt tipidir. Bu bölümde ileri yaşta tanı alan bir PSSSL olgusu sunulmuştur. Yetmiş üç yaşında erkek hasta nörolojik semptomlarla başvurmuş, yapılan biyopsi sonucunda yüksek dereceli B hücreli lenfoma tanısı almıştır. Metotreksat bazlı kemoterapi ile kısmi yanıt elde edilmesinin ardından hastalık kontrolünü artırmak amacıyla tedaviye ibrutinib eklenmiştir. İleri yaşın prognostik önemi ve tedavi seçenekleri literatür doğrultusunda tartışılmıştır. İleri yaş hastalarda performans durumu, komorbiditeler ve tedavi toksisitesi nedeniyle optimal tedavi yaklaşımı halen tartışmalıdır. Bu olgu üzerinden güncel tanı ve tedavi yaklaşımları literatür eşliğinde gözden geçirilmiştir.
