Esansiyel Trombositemiden Sekonder Myelofibroza Dönüşüm: Genç Bir Olgu
Özet
Esansiyel trombositemi (ET), Philadelphia kromozomu negatif myeloproliferatif neoplaziler (MPN) grubunda yer alan, klonal hematopoietik kök hücre hastalığıdır. Hastalık kronik seyirli olmakla birlikte, zaman içerisinde prefibrotik myelofibrozis veya sekonder myelofibrozise progresyon gösterebilir. Bu bölümde, genç yaşta tanı alan, CALR tip 2 mutasyon pozitif ET tanısı ile izlenirken prefibrotik myelofibrozis gelişen ve JAK inhibitörü tedavisi ile takip edilen 25 yaşındaki kadın hasta sunulmuştur. Hastada başlangıçta belirgin trombositoz ve splenomegali mevcut olup, sitoredüktif tedaviye rağmen progresyon bulguları gelişmiştir. Düşük risk skorlarına rağmen hastalık progresyonu göstermesi nedeniyle bu olgu, genç hastalarda ET’nin klinik seyri, moleküler özellikleri, kemik iliği bulguları ve tedavi yaklaşımı açısından dikkat çekicidir.
