Özet

Pulmoner hipertansiyon, ilerleyici pulmoner vasküler yeniden şekillenme ve sağ ventrikül yetmezliği ile karakterize, yüksek morbidite ve mortaliteye sahip kompleks bir hastalık grubudur. Son yıllarda endotelin, nitrik oksit ve prostasiklin yolaklarını hedefleyen tedavilere rağmen hastalığın tamamen kontrol altına alınamaması, yeni terapötik hedeflerin araştırılmasına yol açmıştır. Bu bölümde pulmoner hipertansiyon alanındaki güncel gelişmeler ve geleceğe yönelik yaklaşımlar ele alınmıştır. Özellikle BMPR2/TGF-β sinyal yolunu hedefleyen sotatercept ile başlayan hastalık modifiye edici tedavi dönemi, pulmoner vasküler yeniden şekillenmenin biyolojik mekanizmalarına yönelik yeni bir yaklaşım sunmaktadır. Bunun yanı sıra tirozin kinaz inhibitörleri, serotonin sentez inhibitörleri, immünomodülatör tedaviler, metabolik ajanlar ile gen ve hücre tedavileri gelecek vadeden stratejiler arasında yer almaktadır. Kişiselleştirilmiş tedavi yaklaşımı kapsamında risk sınıflandırması, biyobelirteçler, genetik profilleme ve omik teknolojilerin önemi giderek artmaktadır. Ayrıca yapay zeka destekli analizler, derin fenotipleme yöntemleri ve dijital sağlık uygulamaları tanı, risk değerlendirmesi ve hasta takibinde yeni fırsatlar sunmaktadır. Klinik araştırmalarda ise adaptif çalışma tasarımları, gerçek yaşam verileri, omik teknolojiler ve biyobelirteç temelli yaklaşımlar ön plana çıkmaktadır. Tüm bu gelişmeler, pulmoner hipertansiyon yönetiminin semptom kontrolünden biyolojik mekanizmaların hedeflendiği, daha hassas ve kişiselleştirilmiş bir döneme doğru evrildiğini göstermektedir.

Pulmonary hypertension is a complex group of diseases with high morbidity and mortality, characterized by progressive pulmonary vascular remodeling and right ventricular failure. In recent years, the fact that the disease cannot be fully controlled despite therapies targeting the endothelin, nitric oxide, and prostacyclin pathways has led to the investigation of new therapeutic targets. This chapter addresses current developments and future-oriented approaches in the field of pulmonary hypertension. In particular, the era of disease-modifying therapy that began with sotatercept, which targets the BMPR2/TGF-β signaling pathway, offers a new approach directed at the biological mechanisms of pulmonary vascular remodeling. In addition, tyrosine kinase inhibitors, serotonin synthesis inhibitors, immunomodulatory therapies, metabolic agents, and gene and cell therapies are among the promising strategies for the future. Within the scope of personalized treatment approaches, the importance of risk stratification, biomarkers, genetic profiling, and omics technologies is steadily increasing. Furthermore, artificial intelligence-assisted analyses, deep phenotyping methods, and digital health applications offer new opportunities in diagnosis, risk assessment, and patient follow-up. In clinical research, adaptive trial designs, real-world data, omics technologies, and biomarker-based approaches are coming to the fore. All these developments indicate that the management of pulmonary hypertension is evolving from symptom control toward a more precise and personalized era targeting the underlying biological mechanisms.
