Özet

Bu bölümde pulmoner dolaşımın anatomik ve fizyolojik özellikleri, pulmoner hipertansiyon bağlamında sağ ventrikül yükü, pulmoner vasküler direnç ve egzersiz hemodinamiği ile ilişkilendirilerek ele alınmaktadır. Pulmoner vasküler yatağın düşük basınçlı ve yüksek kompliyanslı yapısı, kapiller rekrutman ve distansiyon mekanizmaları, West zonları, hipoksik pulmoner vazokonstriksiyon ve endotelyal vazoaktif denge temel fizyolojik çerçeveyi oluşturur. Ayrıca sağ ventrikül–pulmoner arter eşleşmesi ve egzersizde mPAP/CO eğimi gibi güncel kavramlar, pulmoner hipertansiyonun klinik değerlendirmesiyle bağlantılı olarak özetlenmektedir.

This chapter reviews the anatomical and physiological features of the pulmonary circulation, relating them to right ventricular load, pulmonary vascular resistance, and exercise hemodynamics in the context of pulmonary hypertension. The low-pressure, high-compliance nature of the pulmonary vascular bed, the mechanisms of capillary recruitment and distension, the West zones, hypoxic pulmonary vasoconstriction, and endothelial vasoactive balance constitute the fundamental physiological framework. In addition, contemporary concepts such as right ventricular–pulmonary arterial coupling and the exercise mPAP/CO slope are summarized in relation to the clinical assessment of pulmonary hypertension.
