Primary lymphedema is a genetic developmental disorder affecting the lymphatic system leading to the accumulation of lymph fluid and resultant swelling, typically in the limbs. Lymphatic fluid collects in the subcutaneous tissues under the epidermis due to obstruction, malformation, or underdevelopment (hypoplasia) of various lymphatic vessels. The diagnosis of primary lymphedema may be confirmed by a thorough clinical evaluation and a variety of specialized imaging tests including lymphoscintigraphy, ultrasound, and magnetic resonance imaging (MRI).  Therapy includes primary physiotherapy and surgery when proper indications exist.  Our case report suggests the reduction surgery is a safe and effective treatment option for patients suffering from severe primary lymphedema that does not respond to conservative treatments, without the need for highly sophisticated equipment or microsurgical expertise.
