Çok Düşük Doğum Ağırlıklı Yenidoğanda Spontan Mide Perforasyonu: Nadir Bir Klinik Olgu Sunumu
Özet
Yenidoğan mide perforasyonu (YMP), çocuk cerrahisi pratiğinde nadir görülen ancak mortalite oranı oldukça yüksek olan cerrahi acillerden biridir. Özellikle 1000 gram altı çok düşük doğum ağırlıklı (VLBW) prematüre bebeklerde bu durum, anatomik immatürite ve eşlik eden sistemik sorunlar nedeniyle çok daha komplike bir hal almaktadır. Bu olgu sunumunda; 27 haftalık gestasyonel yaş ile doğan, postnatal üçüncü günde opere edilen ve operasyon anında vücut ağırlığı 725 gram olan ekstrem prematüre bir bebekte, midenin yaklaşık %50’sinin rezeke edildiği geniş gastrik perforasyon vakasının başarılı cerrahi yönetimi literatür eşliğinde tartışılmaktadır. Erken tanı, doğru cerrahi teknik ve multidisipliner postoperatif bakımın sağkalım üzerindeki kritik rolü vurgulanmıştır.
