Situs İnversus Totalis, Henoch-Schönleın Purpurası ve Opere Spina Bifida Birlikteliği Olan Bir Çocukta Rekürren İnvajinasyon: Nadir Bir Olgu Sunumu
Özet
İnvajinasyon çocukluk çağında intestinal obstrüksiyonun önemli nedenlerinden biridir. İdiyopatik olgular çoğunlukla klasik ileokolik invajinasyon şeklinde görülürken, vaskülitik hastalıklar, anatomik varyasyonlar ve eşlik eden nörolojik sorunlar klinik değerlendirmeyi güçleştirebilir. Bu bölümde, situs inversus totalis tanısı ve spina bifida operasyon öyküsü bulunan, Henoch-Schönlein purpurası gastrointestinal tutulumu zemininde invajinasyon gelişen erkek çocuk olgu sunulmuştur. Başvuruda purpurik döküntü, karın ağrısı ve ultrasonografide invajinasyonla uyumlu bulgular mevcuttu. İlk değerlendirmede eşlik eden karın hassasiyeti, serbest sıvı, intestinal iskemi olasılığı ve akut batın kuşkusu nedeniyle cerrahi eksplorasyon tercih edildi. Operasyonda situs inversus totalis ile uyumlu yerleşim özellikleri izlendi; sol alt kadranda kanlanması bozulmuş ileal ansın spontan redükte olmuş invajinasyon segmenti olabileceği düşünüldü ve konservatif canlılık değerlendirmesi sonrası rezeksiyon yapılmadı. Postoperatif dönemde rekürren invajinasyon gelişmesi üzerine, peritonit ve perforasyon bulgusu olmaması nedeniyle USG eşliğinde hidrostatik redüksiyon başarıyla uygulandı. Olgu, invajinasyon yönetiminde yalnızca standart algoritmanın değil; vaskülitik intestinal tutulum, situs inversusa bağlı ayna anatomisi, geçirilmiş spina bifida cerrahisi ve postoperatif klinik bağlamın birlikte değerlendirilmesi gerektiğini göstermektedir.
