Duodenum Sınırlı Perforasyonu – Beslenme Yolunda Yabancı Cisim

Özet

Yabancı cisim yutulması çocukluk çağında sık görülen gastrointestinal acillerden biridir. Yutulan yabancı cisimlerin büyük çoğunluğu spontan olarak gastrointestinal sistemden geçmesine rağmen uzun, lineer ve radyolusent materyaller ciddi komplikasyonlara neden olabilmektedir.  Yabancı cisim yutulmalarında klinik prezentasyon oldukça değişken olabilir. Bazı hastalarda hiçbir belirti görülmezken bazı hastalarda akut karın tablosu, mediastinit, perforasyon, retroperitoneal inflamasyon veya sepsis gelişebilmektedir. Özellikle radyolusent materyallerde klinik şüphe görüntüleme yöntemlerinden daha değerli hale gelebilmektedir. Bu bölümde, lolipop çubuğu yutulmasına bağlı retroperitoneal duodenal penetrasyon gelişen ve cerrahi uygulanmadan başarılı şekilde tedavi edilen pediatrik olgu sunulmuş, literatür eşliğinde güncel yaklaşım tartışılmıştır. Bu olgu; seçilmiş kendini sınırlandırmış perforasyonlarda minimal invaziv ve cerrahisiz yaklaşımın başarılı olabileceğini göstermiştir.
