Çocukta Kronik Kabızlık İle İlişkilendirilmiş Ve Hirschsprung Hastalığını Taklit Eden Çekum Perforasyonu: Vaka Temelli Öğrenme Yaklaşımı
Özet
Çocuklarda çekum perforasyonu; Hirschsprung hastalığı (HH), enfeksiyon veya inflamatuar bozukluklar gibi altta yatan durumlarla sıklıkla ilişkili olan, nadir görülen ancak hayati tehlike arz eden cerrahi bir acil durumdur. Bu yazıda; mekonyum çıkışında gecikme ve düzenli lavman gerektiren kronik kabızlık gibi Hirschsprung hastalığını düşündüren bir öyküye sahip olan, peritonit ve pnömoperiton ile başvuran 2 yaşında bir erkek çocuk olgusu sunulmaktadır. Acil laparotomide, fekal peritonit ile birlikte izole bir çekum perforasyonu saptandı. Hastaya primer onarım ve diversiyon ileostomisi uygulandı. Kolonun farklı segmentlerinden alınan çok sayıda tam kat biyopside yeterli sayıda ganglion hücresi izlenmesiyle Hirschsprung hastalığı dışlandı. Ameliyat sonrası süreç sorunsuz seyretti ve ileri değerlendirmeler sonucunda fonksiyonel kabızlık tanısı konuldu. Bu olgu, Hirschsprung hastalığı ile fonksiyonel kabızlık arasındaki tanısal ikileme dikkat çekmekte ve yetersiz yönetilen şiddetli fonksiyonel kabızlığın kolon perforasyonu gibi ölümcül komplikasyonlara yol açabileceğini vurgulamaktadır.
