Bu ek bölüm; üst gastrointestinal sistem endoskopisi, kolonoskopi, fleksibl sigmoidoskopi, kapsül endoskopi ve balon enteroskopi gibi temel tanısal endoskopik işlemlere yönelik standart hasta bilgilendirme ve aydınlatılmış onam formlarını içermektedir. Formlar; işlem endikasyonları, hazırlık süreci, sedasyon, olası komplikasyonlar, alternatif yöntemler ve işlem sonrası takip ilkelerini yapılandırılmış biçimde sunmayı amaçlamaktadır. Bölüm, hasta güvenliği, etik yaklaşım ve standardize klinik iletişim ilkeleri doğrultusunda hazırlanmıştır.

The forms aim to present the indications for the procedure, the preparation process, sedation, possible complications, alternative methods, and post-procedure follow-up principles in a structured manner. The section has been prepared in accordance with patient safety, ethical approach, and standardized clinical communication principles.
