Immünosüpresif, Biyolojik ve Küçük Molekül Tedaviler Öncesi Hasta Bilgilendirme
Özet
İmmünsüpresif, biyolojik ve küçük molekül tedaviler; inflamatuvar bağırsak hastalığı (İBH), otoimmün hepatit, primer biliyer kolanjit, IgG4 ilişkili hastalık ve karaciğer nakli gibi durumların yönetiminde modern gastroenteroloji ve hepatolojinin temel taşlarıdır. Bu ajanların en sık ve en kritik yan etkisi, konak savunmasının zayıflamasına bağlı enfeksiyon yatkınlığıdır; ayrıca uzun dönemde malignite ve de novo otoimmün süreç riski taşırlar. Bu nedenle tedaviye başlamadan önce sistematik bir risk değerlendirmesi, latent enfeksiyon taraması (tüberküloz, hepatit B/C, HIV, VZV) ve doğru zamanlanmış aşılama, güvenli tedavinin ön koşuludur. Bu bölümün amacı; bu tedavilere başlanacak erişkin hastalarda ilaç sınıflarının temel özelliklerini, tedavi öncesi risk değerlendirmesini, enfeksiyon tarama ve profilaksi protokollerini, güncel aşılama ilkelerini (canlı/cansız aşı zamanlaması, pnömokok, influenza, hepatit B, zoster) ve hasta bilgilendirmesini standart bir çerçevede sunmaktır. Öneriler ECCO, AASLD, ESsCD ve güncel bağışıklama (ACIP) kılavuzlarına dayanır. Ayrıca standart hasta bilgilendirme metni, sık sorulan sorular ve acil başvuru kriterlerine yer verilmiştir.
Immunosuppressive, biologic and small-molecule therapies are cornerstones of modern gastroenterology and hepatology in inflammatory bowel disease (IBD), autoimmune hepatitis, primary biliary cholangitis, IgG4-related disease and liver transplantation. Their most frequent and critical adverse effect is increased susceptibility to infection due to weakened host defense; they also carry a long-term risk of malignancy and de novo autoimmune processes. Therefore, systematic pre-treatment risk assessment, screening for latent infections (tuberculosis, hepatitis B/C, HIV, VZV) and appropriately timed vaccination are prerequisites for safe therapy. This chapter provides a standardized framework covering drug-class characteristics, pre-treatment risk stratification, infection screening and prophylaxis protocols, current vaccination principles (timing of live/inactivated vaccines, pneumococcal, influenza, hepatitis B, zoster) and patient education, in accordance with ECCO, AASLD, ESsCD and current immunization (ACIP) guidelines, together with standardized patient information, FAQs and emergency criteria.
