Gebelik ve Emzirme Döneminde Gastrointestinal Yakınmalar ve Endoskopik İşlemler

Özet

Gebelik ve emzirme dönemi, gastrointestinal sistem (GİS) hastalıklarının hem klinik seyrini hem de tedavi yaklaşımını köklü biçimde etkileyen fizyolojik bir süreçtir. Bu dönemde en sık karşılaşılan GİS yakınmaları arasında gebeliğe özgü bulantı-kusma (NVP) ve hiperemezis gravidarum (HG), gastroözofagealreflü hastalığı (GÖRH), kabızlık, hemoroid, inflamatuvar bağırsak hastalığı (İBH) alevlenmeleri, intrahepatik kolestaz (ICP), viral hepatitler, Helicobacter pylori enfeksiyonu, akut karın ve biliyer hastalıklar yer almaktadır. Emzirme döneminde ise ilaçların süte geçişi (RID, M:P oranı, Hale kategorileri) ve bebeğe olası etkileri tedavi kararlarını doğrudan şekillendirmektedir.  Tanısal yaklaşımda ultrasonografi ve manyetik rezonans görüntüleme (MR/MRCP) radyasyon içermemesi nedeniyle tercih edilmekte; bilgisayarlı tomografi yalnızca zorunlu durumlarda uygulanmaktadır. Endoskopik işlemler güçlü endikasyon varlığında ve uygun sedasyon protokolleriyle gebelik boyunca güvenle gerçekleştirilebilir; en uygun zaman penceresi ikinci trimesterdir. Farmakolojik tedavide gebeliğin herhangi bir ilacı otomatik olarak kontrendike kılmadığı, aksine kontrolsüz hastalığın fetüs ve anne için ilaçtan daha büyük risk oluşturduğu modern kılavuzların ortak mesajıdır. Emzirme döneminde metotreksat ve mikofenolat mofetil mutlak kontrendikedir; biyolojik ajanlar, 5-ASA preparatları, PPİ'lar ve sık kullanılan GİS ilaçlarının çoğu uygun izlemle sürdürülebilir.

Pregnancy and lactation are physiological states that profoundly alter the clinical course and therapeutic management of gastrointestinal (GI) disorders. The most frequent GI complaints during this period include nausea and vomiting of pregnancy (NVP) and hyperemesis gravidarum (HG), gastroesophageal reflux disease (GERD), constipation, hemorrhoids, inflammatory bowel disease (IBD) flares, intrahepatic cholestasis of pregnancy (ICP), viral hepatitis, Helicobacter pylori infection, acute abdomen, and biliary disease. During lactation, drug-safety assessment — relative infant dose (RID), milk-to-plasma (M:P) ratio, and Hale risk categories — is integral to treatment decisions.  Diagnostic evaluation favors ultrasonography and magnetic resonance imaging given their radiation-free profile; computed tomography is reserved for critical indications. Endoscopic procedures can be safely performed throughout pregnancy with appropriate sedation, with the second trimester representing the optimal window. A fundamental principle endorsed by current guidelines is that untreated disease poses greater risk to the mother and fetus than most therapeutic agents; methotrexate and mycophenolate mofetil are absolutely contraindicated during breastfeeding, whereas biologics, 5-ASA compounds, proton pump inhibitors, and most common GI medications can be continued with appropriate monitoring.
