Peptik Ülser Hastalığı
Özet
Peptik ülser hastalığı, mide veya duodenum mukozasında asit-peptik hasar sonucu gelişen, sık görülen ve komplikasyon geliştiğinde önemli morbidite ve mortaliteye yol açabilen bir hastalık grubudur. Hastalığın başlıca nedenleri H. pylori enfeksiyonu ve nonsteroid antiinflamatuvar ilaç kullanımıdır; bu nedenle hasta bilgilendirmesinde etiyolojinin belirlenmesi, eradikasyon tedavisine uyum, gereksiz analjezik kullanımından kaçınma ve tedavi sonrası doğrulama testinin önemi vurgulanmalıdır. Klinik değerlendirmede dispeptik yakınmaların süresi, kanama riski, ilaç öyküsü ve malignite olasılığını artıran özellikler birlikte ele alınmalıdır. Hematemez, melena, açıklanamayan demir eksikliği anemisi, istemsiz kilo kaybı, progresif disfaji, persistan kusma, senkop veya hemodinamik instabilite gibi alarm bulguları varlığında gecikmeden ileri değerlendirme ve gerekli durumlarda acil başvuru önerilmelidir. Bu bölümün amacı, peptik ülser hastalığında hekimlerin hastaya aktaracağı temel mesajları bilimsel kanıta dayalı, anlaşılır ve standartlaştırılmış bir çerçevede sunmak; tanı, tedavi, izlem ve acil başvuru gerektiren durumlar konusunda klinik pratiğe uygulanabilir bir hasta bilgilendirme modeli oluşturmaktır.
Peptic ulcer disease is a common group of disorders characterized by acid-peptic injury of the gastric or duodenal mucosa and may lead to substantial morbidity and mortality when complications occur. Major etiological factors are Helicobacter pylori infection and nonsteroidal anti-inflammatory drug use; therefore, patient education should emphasize identification of the underlying cause, adherence to eradication therapy, avoidance of unnecessary analgesic use, and the importance of post-treatment confirmation testing. Clinical assessment should integrate the duration of dyspeptic symptoms, bleeding risk, medication history, and features that may increase the likelihood of malignancy. In the presence of alarm symptoms such as hematemesis, melena, unexplained iron deficiency anemia, unintentional weight loss, progressive dysphagia, persistent vomiting, syncope, or hemodynamic instability, prompt further evaluation and, when necessary, urgent medical assessment should be recommended. The aim of this chapter is to provide physicians with an evidence-based, comprehensible, and standardized framework for conveying the key messages of peptic ulcer disease to patients and to establish a clinically applicable patient information model regarding diagnosis, treatment, follow-up, and situations requiring urgent medical attention.
