Reflü Yakınmaları
Özet
Gastroözofageal reflü hastalığı, mide içeriğinin özofagusa geri kaçışı sonucunda hastada rahatsız edici semptomlara veya mukozal komplikasyonlara yol açabilen, klinik pratikte sık karşılaşılan bir üst gastrointestinal sistem hastalığıdır. Tipik yakınmalar arasında retrosternal yanma ve regürjitasyon ön planda yer alırken, göğüs ağrısı, kronik öksürük, ses kısıklığı ve boğaz semptomları gibi ekstraözofageal bulgular da klinik değerlendirmede dikkate alınmalıdır. Reflü yakınmaları çoğu hastada yaşam tarzı düzenlemeleri ve asit baskılayıcı tedavi ile kontrol altına alınabilir; ancak disfaji, odinofaji, gastrointestinal kanama, açıklanamayan kilo kaybı, persistan kusma, demir eksikliği anemisi veya malignite şüphesi gibi alarm bulgularının varlığı ileri değerlendirme gerektirir. Hastaya aktarılması gereken temel mesaj, reflünün sık görülen ve çoğu zaman yönetilebilir bir durum olduğu; bununla birlikte uzun süreli, tedaviye dirençli veya alarm bulgularıyla seyreden yakınmaların ihmal edilmemesi gerektiğidir. Bu bölümün amacı, reflü yakınmaları olan bireylerde kanıta dayalı klinik yaklaşımı özetlemek, tanı ve tedavi basamaklarını hasta bilgilendirme perspektifiyle birleştirmek ve hekim-hasta iletişiminde kullanılabilecek standart, anlaşılır ve güvenilir bir bilgilendirme protokolü sunmaktır.
Gastroesophageal reflux disease is a common upper gastrointestinal disorder encountered in clinical practice, which may lead to troublesome symptoms or mucosal complications as a result of the reflux of gastric contents into the esophagus. While retrosternal burning and regurgitation are the predominant typical complaints, extraesophageal manifestations such as chest pain, chronic cough, hoarseness, and throat-related symptoms should also be considered during clinical evaluation. In most patients, reflux-related complaints can be controlled with lifestyle modifications and acid-suppressive therapy; however, the presence of alarm features such as dysphagia, odynophagia, gastrointestinal bleeding, unexplained weight loss, persistent vomiting, iron deficiency anemia, or suspicion of malignancy requires further evaluation. The key message to be conveyed to patients is that reflux is a common and often manageable condition; nevertheless, long-standing, treatment-resistant complaints or symptoms accompanied by alarm features should not be overlooked. The aim of this chapter is to summarize the evidence-based clinical approach to individuals presenting with reflux-related complaints, to integrate diagnostic and therapeutic steps with a patient education perspective, and to provide a standardized, clear, and reliable information protocol that can be used in physician-patient communication.
