Kalsifik RCA Lezyonunda Stent Açılmaması ve OPN Balon Kullanımı

Özet

Koroner arter kalsifikasyonu, perkütan koroner girişim (PKG) sırasında stent açılmasını güçleştiren ve olumsuz klinik sonuçlarla ilişkili önemli bir klinik sorundur. Standart non-compliant (NC) balonların yetersiz kaldığı 'undilatable' kalsifik lezyonlarda özel lezyon modifikasyon araçlarına ihtiyaç duyulmaktadır. OPN NC süper yüksek basınçlı balon (SIS Medical AG,Fraunfeld,Switzerland), patentli çift katmanlı  poliyamid teknolojisi sayesinde 35 atmosfer (ATM) nominal patlama basıncı ve 42 atmosfer(ATM ) fabrika test kapasitesiyle bu boşluğu doldurmaktadır. Çift katman yapısı; uniform ekspansiyon, düşük ‘Köpek Kemiği’(dog-boning) etkisi ve rüptür halinde hidrolik koruma sağlamaktadır. Temel endikasyonları; sıkı kalsifik lezyonlarda predilatasyon ihtiyacı, stent içi restenoz ve tam stent açıklığı sağlanamaması durumudur.  Bu vakamızda    62 yaşında stabil anjina pektoris tanısı ile koroner anjiyografi yaptığımız  erkek hastamızda standart predilatasyon işlemlerinden sonra 3,5x32 mm DES 18 ATM  basınç altında implante edildi. Stent ortasında yeterli açıklık sağlanamadığı görülünce 4,0x15 mm  NC balon ile 20 ATM  basınç altında stent içine ilave dilatasyon yapıldı. Ancak başarılı olunamayınca 3,5x15 mm  OPN    Balon ile 35 ATM de yüksek basınç ile postdilatasyon yapılarak  başarılı şekilde tam stent açıklığı sağlandı.Komplikasyon olmadı.

Coronary artery calcification remains a major challenge in percutaneous coronary intervention (PCI), as it frequently impairs optimal stent expansion and is associated with adverse procedural and long-term clinical outcomes. In heavily calcified, undilatable coronary lesions where conventional non-compliant (NC) balloons fail to achieve adequate lesion preparation, dedicated lesion-modifying devices are often required. The OPN NC super high-pressure balloon (SIS Medical AG,Frauenfeld, Switzerland) was specifically developed to address this therapeutic gap. Its patented twin-layer polyamide balloon technology enables inflation to a rated burst pressure of 35 atm, with a factory-tested pressure resistance of up to 42 atm. The dual-layer design provides uniform balloon expansion, minimizes the dog-boning effect, and offers hydraulic protection in the event of balloon rupture, thereby enhancing procedural safety. The principal indications for the OPN NC balloon include undilatable de novo calcified lesions, in-stent restenosis, and stent underexpansion. We present the case of a 62-year-old man with stable angina pectoris who underwent coronary angiography, which demonstrated a severely calcified coronary lesion. Following conventional lesion predilatation, a 3.5 × 32 mm drug-eluting stent (DES) was implanted at 18 atm. Control angiography revealed inadequate expansion of the mid-portion of the stent. Post-dilatation was initially performed using a 4.0 × 15 mm NC balloon inflated to 20 atm; however, satisfactory stent expansion could not be achieved. Subsequently, a 3.5 × 15 mm OPN NC super high-pressure balloon was used for post-dilatation at 35 atm, resulting in complete stent expansion with an optimal angiographic outcome. No procedural or post-procedural complications were observed. This case highlights the clinical utility of the OPN NC super high-pressure balloon as an effective and safe bailout strategy for achieving optimal stent expansion in severely calcified coronary lesions refractory to conventional NC balloon dilatation. The device may represent a valuable therapeutic option in selected complex PCI procedures where standard balloon technology fails to provide adequate lesion modification.
