Özet

Kounis sendromu, alerjik veya anafilaktik reaksiyonlar sırasında gelişebilen ve akut koroner sendromu taklit eden nadir bir klinik sendrom tablosudur. Patofizyolojisinde mast hücrelerinden salınan inflamatuar mediyatörlerin neden olduğu koroner vazospazm, koroner plak rüptürü veya tromboz yer alır. Hastalık Tip I, II ve III olarak sınıflandırılır. Olguda, tetanos aşısı sonrası gelişen anafilaktik şok ile eş zamanlı ST segment elevasyonu ve troponin yüksekliği mevcuttur. Normal koroner anjiyografi bulgularının eşlik etmesi nedeniyle, aşırı duyarlılık reaksiyonunun koroner olayı tetiklediğini düşündürmektedir. Bu durum, Tip I Kounis sendromu ile uyumludur. Erken tanı ve uygun tedavi, hem alerjik reaksiyonun hem de kardiyak komplikasyonların yönetiminde hayati önem taşımaktadır

Kounis syndrome is a rare clinical condition that can occur during allergic or anaphylactic reactions and may mimic acute coronary syndrome. Its pathophysiology involves coronary vasospasm, coronary plaque rupture, or thrombosis induced by inflammatory mediators released from activated mast cells. The syndrome is classified into Types I, II, and III. In the present case, the patient developed anaphylactic shock following tetanus vaccination, accompanied by simultaneous ST-segment elevation and elevated cardiac troponin levels. The presence of normal coronary angiography findings suggests that the coronary event was triggered by the hypersensitivity reaction. These findings are consistent with Type I Kounis syndrome. Early recognition and appropriate management are crucial for improving outcomes by ensuring timely treatment of both the allergic reaction and its associated cardiac complications.
