Refrakter Ventriküler Taşiaritmide Ultrasonografi Kılavuzluğunda Stellat Gangliyon Bloğu Tedavisi
Özet
Elektriksel fırtına ile seyreden refrakter ventriküler taşiaritmiler yüksek mortalite ile ilişkilidir ve konservatif tedavilere dirençli olabilir. Stellat gangliyon bloğu algoloji hekimleri tarafından baş ve üst ekstremitenin sempatik kaynaklı ağrılarının tedavisinde sıklıkla kullanılmaktadır. Ventriküler taşikardi tedavisinde de kardiyak sempatik aktiviteyi baskılayarak etki gösterdiği bilinmektedir. Bu olgu sunumunda, medikal tedavi ve tekrarlayan şoklamalara rağmen dirençli seyreden ventriküler taşikardinin ultrasonografi eşliğinde uygulanan sol stellat gangliyon bloğu ile tedavisini sunuyoruz. Sol stellat gangliyon bloğu, refrakter ventriküler taşikardide etkili ve minimal invaziv bir köprü tedavi seçeneği olarak öne çıkmaktadır.
Refractory ventricular tachyarrhythmias presenting as an electrical storm are associated with a high mortality rate and may not respond to conventional treatments. Stellate ganglion block is used by pain management specialists to treat sympathetic-origin pain in the head and upper extremities. It is also known to be effective in the treatment of ventricular tachycardia by suppressing cardiac sympathetic activity. In this case report we present the treatment of refractory ventricular tachycardia which persisted despite medical therapy and repeated defibrillation, using a left stellate ganglion block performed under ultrasound guidance. This procedure should be considered as an effective and minimally invasive bridge therapy option for patients with refractory ventricular tachycardia.
