Bifürkasyon Lezyonlarında Double Kissing Crush Tekniğinin Olgu Üzerinden Anlatımı
Özet
Koroner bifürkasyon lezyonları kateter laboratuvarlarında yaklaşık %15-20 oranında karşılaşılan, bir ana dal ve bir yan dalı içine alan daralma veya tıkanıklıklardır. Girişimsel tedavileri daha kompleks işlemleri gerektirir. T stentleme, T ve küçük çıkıntı (TAP), culotte, klasik crush, minicrush, double kissing (DK) crush teknikleri çift stent tekniklerinden bazılarıdır. Bunlar içerisinde en sık kullanılan ve provizyonel stentlemeye üstünlüğü gösterilen teknik DK crush tekniğidir. Biz bu yazıda DK crush tekniğini polikliniğimize başvuran 62 yaş erkek hastaya yaptığımız koroner anjiografi sonucunda sirkumflex (CX) arterde saptadığımız bifürkasyon lezyonu üzerinden basamak basamak anlatmayı amaçladık.
Coronary bifurcation lesions, characterized by narrowing or occlusion involving a main branch and a side branch, are encountered in approximately 15-20% of cases in catheterization laboratories. Interventional treatments for these lesions require more complex procedures. Among the various double-stenting techniques are T-stenting, T and small protrusion (TAP), culotte, classical crush, mini-crush, and double kissing (DK) crush. Of these, the DK crush technique is the most widely used and has demonstrated superiority over provisional stenting. In this report, we aim to provide a step-by-step description of the DK crush technique through a case involving a bifurcation lesion detected in the circumflex (CX) artery of a 62-year-old male patient who presented to our outpatient clinic.
