Kanser Tedavisinde Tanşinon-Iıa İle Emt’nin Hedeflenmesi

Özet

Kanser, dünyada mortalite ve morbiditenin başlıca nedenlerinden birisidir. Kanser ve metastaz süreçlerinin anlaşılması, yeni tedavi stratejilerinin geliştirilmesi açısından büyük önem taşımaktadır. Epitelyal-mezenkimal geçiş (EMT), kanser hücrelerinin invazyon, metastaz ve kemoterapi direnci gelişiminde kritik rol oynar. Son yıllarda doğal kaynaklı bileşiklerin kanser tedavisindeki etkileri araştırılmaktadır. Salvia miltiorrhiza bitkisinden elde edilen Tanşinon-IIA, lipofilik bir diterpen kinon yapısına sahiptir ve antioksidan, antiinflamatuvar, kardiyoprotektif ve antikanser etkileri literatürde bildirilmiştir. Yapılan çalışmalar, Tanşinon-IIA’nın çeşitli kanser türlerinde hücre proliferasyonunu baskıladığını, apoptozu indüklediğini, anjiyogenez, invazyon ve metastazı azalttığını göstermektedir. Özellikle EMT sürecini hedefleyerek E-kaderin ekspresyonunu artırdığı; N-kaderin, vimentin, Snail ve Slug gibi mezenkimal belirteçlerin ekspresyonunu azalttığı bildirilmiştir. Ayrıca STAT3-CCL2 ve PI3K/Akt/ERK gibi sinyal yolaklarını düzenleyerek EMT’yi baskıladığı ve böylece tümör ilerlemesini, metastazı ve ilaç direncini azalttığı ortaya konmuştur. Bununla birlikte, biyoyararlanımının düşük olması ve potansiyel toksisite riskleri klinik kullanımını konusunda sınırlamalara yol açmaktadır. Bu kitap bölümünde doğal bir fitokimyasal ajanlardan biri olan Tanşinon-IIA’nın, EMT’yi hedeflemesi yoluyla kanser tedavi süreçlerine ve klinik uygulamalara yönelik etkisinin araştırılmasının önemi vurgulanmıştır.

Cancer is one of the leading causes of mortality and morbidity worldwide. Understanding the processes of cancer development and metastasis is important for the development of new therapeutic strategies. Epithelial-mesenchymal transition (EMT) plays a critical role in cancer cell invasion, metastasis, and the development of chemoresistance. In recent years, the effects of naturally derived compounds in cancer therapy have attracted considerable research interest. Tanshinone-IIA, obtained from the plant Salvia miltiorrhiza, is a lipophilic diterpene quinone compound. Its antioxidant, anti-inflammatory, cardioprotective, and anticancer properties have been reported in the literature. Studies have demonstrated that Tanshinone-IIA suppresses cell proliferation, induces apoptosis, and reduces angiogenesis, invasion, and metastasis in various types of cancer. It has been reported to target the EMT process by increasing E-cadherin expression while decreasing the expression of mesenchymal markers such as N-cadherin, vimentin, Snail, and Slug. Furthermore, Tanshinone-IIA was shown to inhibit EMT by regulation of signaling pathways including STAT3-CCL2 and PI3K/Akt/ERK, thereby reducing tumor progression, metastasis, and drug resistance. However, its low bioavailability and potential toxicity risks pose limitations to its clinical application. This book chapter emphasizes the importance of Tanshinone-IIA, a natural phytochemical agent, on cancer treatment and clinical applications through its ability to target EMT.
