Sodyum Nitrit Zehirlenmesi ve Dirençli Siyanoz: Olgu Sunumu ve Literatür Derlemesi
Özet
Sodyum nitrit, gıda koruyucusu olarak kullanılan ve son yıllarda internet üzerinden özkıyım amaçlı temin edilerek alımı artan toksik bir maddedir. Sodyum nitrit maruziyeti, hafif baş ağrısı ve siyanozdan nöbet, koma, kardiyovasküler çöküş ve ölüme kadar değişen geniş bir klinik tabloya yol açabilir. Bu olgu sunumunda, özkıyım amaçlı sodyum nitrit alımı sonrası acil servise başvuran 33 yaşında kadın hasta sunulmaktadır. Hastanın başvurusunda 8 L/dk oksijen tedavisine rağmen periferik kapiller oksijen satürasyonunun %85 düzeyinde kalması, fizik muayenesinde belirgin siyanoz saptanması, arter kan gazı numunesinin çikolata renginde olması dikkat çekiciydi. Arteriyel kan gazı analizinde parsiyel oksijen basıncı 141 mmHg ölçülerek satürasyon açığı tespit edilmiş ve methemoglobinemi ön tanısı konulmuştur. Acil servis takibinde tonik-klonik nöbet geçiren ve hava yolu güvenliğini sağlayamadığı için entübe edilen hastaya 1 mg/kg intravenöz metilen mavisi uygulanmıştır. Tedavi sonrası siyanozu gerileyen ve yoğun bakım ünitesinde takibe alınmıştır. Bu olgu, açıklanamayan hipoksi ve satürasyon açığı olan hastalarda sodyum nitrit zehirlenmesinin akılda tutulmasının önemini vurgulamaktadır.
