Haloperidol İntoksikasyonuna Bağlı Ventriküler Taşikardinin Tedavisinde Lipid Emülsiyonu: Olgu Sunumu
Özet
Haloperidol, yaygın olarak kullanılan bir antipsikotik olmakla birlikte, yüksek doz maruziyetlerinde ciddi kardiyotoksisiteye yol açabilir. En sık bildirilen aritmik komplikasyon QT uzaması ve torsades de pointes olmakla birlikte, nadiren ventriküler taşikardi gibi yaşamı tehdit eden ritim bozuklukları da görülebilir. Bu tür olgularda standart tedavi yaklaşımlarına rağmen refrakter seyir izlenebilir. Bu bölümde, yüksek doz haloperidol alımına bağlı gelişen ve tekrarlayan kardiyoversiyon ile medikal tedavilere yanıt vermeyen ventriküler taşikardinin intravenöz lipid emülsiyonu tedavisi ile hızla düzeldiği 22 yaşında bir kadın olgu sunulmaktadır. Hastanın yakın zamanda moksifloksasin kullanmış olması, aritmojenik riskin artmasına katkıda bulunmuş olabilir. İntravenöz lipid emülsiyonu, başlangıçta lokal anestezik sistemik toksisitesi için geliştirilmiş olmakla birlikte, günümüzde lipofilik ilaçlara bağlı refrakter kardiyotoksisite olgularında giderek daha fazla kullanılmaktadır. Bu olgu, lipid emülsiyonunun haloperidol gibi non-lokal anestezik ajanlara bağlı kardiyotoksisite durumlarında da etkili bir tedavi seçeneği olabileceğini göstermektedir.
