Uterin Rüptür Olarak Gelişen Sezaryen Skar Gebeliği: Bir Olgu Sunumu

Özet

Sezaryen skar gebeliği (SSG), modern obstetride maternal morbiditeyi tehdit eden ve görülme sıklığı giderek artan ciddi bir ektopik gebelik formudur. Küresel sezaryen oranlarındaki artışa paralel olarak izlenen bu ektopik gebelik türü, erken evrede tanınmadığında maternal hayatı doğrudan tehdit etmektedir. Bu nedenle, masif hemoraji ve rüptür gibi yüksek mortaliteli komplikasyonların önüne geçebilmek için erken teşhis ve doğru yönetim stratejileri hayati önem taşır. Sunduğumuz bu vaka, SSG yönetiminde hasta uyumunun ne kadar belirleyici olduğunu ve komplike durumlarda acil cerrahi müdahalenin kaçınılmazlığını vurgulamaktadır. Bu olgu sunumunda, 7. gebelik haftasında tanı konulmasına rağmen tedaviyi reddeden ve 31. haftada akut uterin rüptür tablosuyla başvuran bir hastanın yönetimi ele alınmaktadır. Karın ağrısı şikayetiyle başvuran hastada, plasenta inkreata  zemininde gelişen rüptür ve intrauterin ölü fetus nedeniyle acil sezaryen doğum yapılan ve ameliyat sırasında masif kanama nedeniyle abdominal histerektomi gerçekleştirilmiştir. Bu vaka, SSG yönetiminde erken tanının yanı sıra hasta eğitimi ve tedaviye uyumun hayati önemini; komplikasyonların önlenmesinde ise yakın klinik takibin vazgeçilmezliğini çarpıcı bir şekilde ortaya koymaktadır.
