İstilacı Böbrek: Olgu Sunumu
Özet
Polikistik Böbrek Hastalığı, böbreklerde kist gelişimi ve ilerleyici parankimal hasar ile seyreden genetik bir nefropatidir. Hastalık genellikle renal filtrasyon kapasitesinin azalması sonucu kronik böbrek yetmezliğine ilerler. Literatürde nadir görülen ancak hayati önem taşıyan bir komplikasyon ise, devasa boyutlara ulaşan kistlerin üreterler üzerinde oluşturduğu dış bası (kitle etkisi) sonucu gelişen postrenal akut böbrek yetmezliğidir (ABY). Bu sunumda; hipertansiyon ve anüri ile başvuran, batın içini adeta istila eden kistik yapıların yol açtığı obstrüktif üropati ve buna eşlik eden ağır metabolik tabloya sahip genç bir olgu tartışılmaktadır. Acil serviste yapılan değerlendirmede olguya postrenal ABY tanısı konulmuş ve uygulanan tedavi adımları sonucunda hastalığın seyri sunulmuştur.
