Omuz Çıkığında Tanıdan Redüksiyona Klinik Yol Haritası: Olgu Sunumu Eşliğinde Acil Yaklaşım
Özet
Akut omuz çıkıkları, acil serviste sık karşılaşılan ve hızlı tanı ile uygun tedavi gerektiren klinik durumlardır. Geniş hareket açıklığına sahip glenohumeral eklem, anatomik özellikleri nedeniyle instabiliteye yatkındır ve omuz çıkıkları sıklıkla şiddetli ağrı, fonksiyon kaybı ve potansiyel nörovasküler yaralanmalarla birliktedir. Bu bölümde, akut omuz çıkığı ile acil servise başvuran bir olgu üzerinden, tanıdan redüksiyona kadar izlenmesi gereken klinik yol haritası sunulmuştur. Olgunun değerlendirilmesinde ayrıntılı anamnez, sistematik fizik muayene ve redüksiyon öncesi–sonrası nörovasküler değerlendirmenin önemi vurgulanmıştır. Direkt omuz grafileri ile çıkık yönü belirlenmiş, eşlik eden kırıklar dışlanmış ve uygun analjezi altında kapalı redüksiyon başarıyla uygulanmıştır. Redüksiyon sonrası klinik ve radyolojik değerlendirme ile eklem uyumunun sağlandığı doğrulanmıştır. Olgu temelli bu yaklaşım, akut omuz çıkıklarında posterior çıkıkların atlanmaması, uygun redüksiyon tekniğinin seçilmesi ve komplikasyonların erken tanınması açısından sistematik bir rehber sunmaktadır.
